We have researched our off site storage records, have retrieved these records, have reviewed other records which were deemed not to belong to this study and have transferred the appropriate records to Applied Epidemiology, Inc. (AEI) per your request. During this search, ENSR has not been able to locate the death certificates for this study. ENSR had also researched its files of data tapes and have found the master file for the study. Because of agreements made between ENSR and various states and jurisdictions regarding confidentiality of death certificates, CMA has requested in writing from each state with such an agreement to allow ENSR to transfer the respective death certificates to Applied Epidemiology, Inc. Forty-two states were identified that needed a written agreement to allow ENSR to transmit the death certificate information. ENSR was then to provide AEI with the name and state file number so that AEI could obtain the death certificates from the respective state. Based on discussions with the various states that have not yet responded in writing, you have been assured that all but New York City and New York State will agree with the death certificate transfer. Without the death certificates, ENSR cannot delete the specific deaths from the states that will not agree to the transfer process. The data tapes do not list state or location of death so there Is no feasible method to separate the data. CMA understands that ENSR cannot do with what it does not have. CMA wants this study to be completed; thus, CMA is authorizing ENSR to release the master data file to AEI acknowledging that the wishes of some states and jurisdictions cannot be met without damaging the master data file by removing all death certificate information regarding the coded underlying causes of death. ENSR will do so before transmitting the master data file to AEI. if the death certificates cannot be found, ENSR will transmit the master data file, as it is, to AEI. Both CMA and ENSR recognize that the former principal Investigator, Otto Wong, Sc.D., opposes this transfer without the signed release forms.
