Robinson SHAC The First Quarter Safety and Housekeeping inspection for HCN Complex was conducted by the Inspection Team of George Bostick, Jack Richards, Tommy Holder, Harry Robinson and B. J. Sanders on 1/29/85. Overall the housekeeping looked good. The following items need attention: SAFETY 1. Utility station near 25E1-1 has sign missing from steam line. 2. Utility station on second floor near 25V2-1 has sign missing from water line. 3. Manlift was in use on west side of C.R. with no warning signs out. 4. New instrument box mounted on a post on the north side of TBA at ground level needs to be painted for "head knocker" alert. 5. Ladder improperly stored on east side of TBAwithout inspection tag. 6. The 25E1-2 piping system has many flange sprayguards missing. Since the walls have been removed from this area, it is possible that the policy for using flange spray should be looked at again. 7. Five minute escape mask on second floor of TBA (west side) needs a new storage case and a better mounting method. 8. Utility station on second floor of TBA (west side) needs sign on water line. Utility station, same level on east side has loose steam hose connection at the block valve. 9. Utility hoses without pins near the caustic area. 10. Steam hose at steamout area out of date. 11. Steam hose out of date near 27T6. 12. Tank truck of caustic being unloaded without using wheel chocks or ground wire. SC 000355 LAM001552 13. Phosphoric acid tank truck spot has no ground wire. HOUSEKEEPING 1. Several ladders around the units were improperly stored. A lot of work was underway during the inspection and possibly these ladders had only recently been in use. 2. The area north of the maintenance shop should be cleaned and straightened up. It is cluttered with pipe, tools, ladders, etc. OTHER COMMENTS 1. A very large number of crowsfoot and nitrogen connections were permanently installed on process and instrument lines all over the units. The inspection team questioned this and were given the answer that this practice is acceptable. Many of the fittings were corroded, no washers and generally unusable. 2. Should hearing protection be required while steaming out equipment? 3. A concern was expressed over lack of respect for "blue light" warning during spills or line blowing. 4. A good daily log was maintained for updates, changes, concerns, etc. 5. Attitude was good toward improvement of the units. Please respond in writing to J.
