Sanborn visited the 4D Oil and Gas, LLC oil and gas facility located at latitude 36.808105 and longitude -96.281125 near Pawhuska, Oklahoma on 10/02/2019 for an unannounced inspection. There were no representatives of 4D Oil and Gas, LLC present for the inspection. The inspection was conducted by authority of the Clean Water Act and the Safe Drinking Water Act to determine compliance with the Environmental Protection Agency (EPA) regulations. FACILITY DESCRIPTION The facility consisted of the injection well with the EPA inventory number: OS 5571 and had an associated tank and pump. Section II - OBSERVATIONS I observed that the electric brine pump leaked into the secondary containment pit. I saw some surface damage west of the tank battery and well. There was about one foot of water in the pit during the inspection. Section III - AREAS OF CONCERN The pit had fluids in it. Section IV - FOLLOW UP EPA will conduct a follow up inspection in approximately 3 months. Section V - LIST OF APPENDICES N/A 2
