(Please refer to the specific recommendations beginning on page 6 of the referenced report) . 5. 'install manually controlled inert gas supply to snuff fires within hopper system." Please advise me as to the desired location of the manual valve. 7. "Review and revise hopper design as required to prevent large product holdup in hopper areas." It was agreed that due to the difficulty in removing the hoppers and chutes, electric vibrators would be added to the trans fer system to aid in'the movement of biphenyl flake. One vibrator would be placed near the diverter gate, ` and vibrators as needed (one or two) on the converging chute directly under the flaker. 8. 'provide alternate escape route from second and third levels of the packaging area. It is understood that an addition to CEA 1430 has been designed to provide an escape route from the third level. It is recommended that the existing proposal be reviewed to incorporate a means of escape from the second level." An escape ladder and associated platform has been provided by CEA 1430 and delivered to the Plant. This ladder is to be installed by the plant based upon prior agreements. CED will design an escape route from the second level. This will be in the form of either a new ladder directly to the floor or mo'difications to the second level to allow stairs to the aroclor feed tank platform. ' 9. "provide adequate door design for both Biphenyl and Aroclor flakers which meet operational needs and curtail product and vapor loss from the flaker systems.11 10. "Review operation and design of dust collection system to eliminate spillage of product on second and third level of flaking area." It is believed that the recently installed modification! to the dust collection system will eliminate the spillage of product at the second and third level of the flaking area. These items will be handled separately from CEA 1430, A copy of the Near-Miss Accident Report has been forwarded to R. J. Green, CED Safety Manager for CEA 1430. Mr. Green should be contacted by Anniston T.S.D. as they discharge their responsibility under item No. 6 -"develop adequate background information to predict and control explos-ion-fire potential of the biphenyl packaging system.'
