This procedure needs to be formalized and instructions given to lead operators. - MPB 8 JEB An investigation into a possible level device to prevent reactor overfilling is to be done by MPB. 3. Expedite delivery of amp meter for D-400. - JAD 4. Investigate relocation of remote camera or revision to swirl piping to allow monitoring by lead operator. - JAD 5. Estimate cost to install amperage recorders on each reactor to serve as backup. - JAD 6. Determine quantity of VCM emmission from available data. - RAF 7. Obtain all data necessary for EPA report. - RAF '8. A detailed investigation and write-up is to be done by CRM, JEB and MPB. Qj Make recommendations of possible solution to prevent reoccurance.
