Before 'agreeing to^pirticipate in this study, it is important.that you understand. the_pdfpbse* of the examination, its benefits and possible. discomforts. The objective of this thorough examination is to determine the health status of the employees and former employees of the Monsanto Industrial Chemical Company in Nitro, West Virginia and to identify health problems which may be related to the occupational environment. The examination will focus on possible health effects over a long period of time of chemicals associated with the making of 2,4,5-T such as dioxin.
If __________________________________________________ , agree to participate . in the medical research study conducted by the University of Cincinnati under the direction of Dr. Raymond R. .Suskind. I understand that'l^will be interviewed and asked a series of questions about myself and the health of my family, in addition to a physical examination I shall permit chest X-rays and an EKG to be done; shal1.permit pulmonary function and nerve conduction tests to be performed; shall have blood drawn and furnish urine for laboratory examination.
