RE: PROPOSED REVISIQIS TO THE ACCIDENT PREVENTION CRUDE
The following Items need to be revised In the Accident Prevention
Guide.
1. The compressed gas cylinder, Item 2 on Page 5> needs to be adapted or interpreted to meet the needs of the Smelting Mechanical Maintenance and Potroom Utility Departments. Both of the departments transport cDepressed gas cylinders with the caps off and the gauges and hoses connected to the gas cylinders.
2. We need to make provisions In the yellow tag and danger tag rules to allow millwrights to use a plug-in type jog switch while the switch box is tagged out when changing buckets on elevators (Pages 11-15)
3. The minimum clearance between a ladder and a structure should be seven inches instead of six inches as printed on Page 26.
It. Item 6 on Page 35 needs clarification.
5. The Smelting Mechanical Maintenance Department does not have a list of aerating equipment as implied by Item 7 on Page 37*
6. Item 2 on Page 47 should read "Sever enter a mixer unless the switch is off, the equipment danger tagged, the drive disconnected, and you have a signed permit from your supervisor to enter the mixer."
Please inform me of your proposed action on the items listed.
Items 1 and 2 of this letter are urgent as they pertain to daily operation
in the plant area, and depending upon the interpretation, we may be violating
established safety guidelines.
