" His chief complaints were shortness of

He admitted, however, to vtinte cough for approximately six months - h was productive oi yellow mucus. Tltere : also two bouts of hemoptysis of minor degree and six months prior to entry. There was a JO lb. weight loss over a six-month
period .iiul considerable fatigue. He also noted
clubbing of his lingers for "some years."
For JO years the patient Itad worked actively
as an asbestos insulator, working both inside and
out and in new and old buildings. Tins entailed
cutting, molding, and applying asbestos insulation
tu pipes, boilers, etc. At no time liad lie or ills
associates worn any protective mask. He had
worked full-time until one week prior to hos
pitalisation. He also had considerable contact with
l-'iherglas. He had not smoked in the past five
}<ars. lut prior to tltat he had smoked three
lvk:icc* of cigarettes daily tor many years. There
wa. a moderately heavy alcohol intake, primarily
wine, in the past six months
Family history was negative ior malignancy
There was contact 20 years previous with a brother
who had tuberculous. -
I'liv Meal examination revealed an extremelv
vm.iviaicd and dyspncic whitc man. appearing
ilironically ill. The blood pressure was 140.100
In Ivth arms, pulse 84 and regular, temperature
IU04 rcctally, respiration 24. height - ft 10 in.,
weight I22b$ lb.
There was marked clubbing of the fingers and
trx> liut no cyanosis. There was I-laiera I arcus
senilis. No abnormalities were noted in the neck.
The chest showed rerlueesl excursion in l->th lunc
held*. Dullness arxl loth ileeteased hrraili >uuipU
and fremitus were noted on tlie right involving
the entire lower halt of the lung field Medium dry
rales which did not clear seerc heard anteriorly and
posteriorly in the interior areas of loth lung>. The
heart was not enlarged to percussion or palpation,
ami no murmurs were present. There was a split
first staind at the apex Complete examination
otherwise was within normal limits.
Laboratory studies on entry showed a Hg of

morphonuclears 78. nonsegmemers 7. lymphocytes
l1), monocytes 2, and eosinophils 1; FBS 112 nig.;
RL'N 16 mg.; serum albumin J.5 cm ; globulin
J.l gm.; thymol turbidity, arid phospliatase. direct
ami indirect bilirubin, amylase, ami clccirolyte
panel -were normal. The alkaline phoapliatase was
6.5 and 5.9 Boilantky units 1X1 two occasions.
Urines shewed persistent 1+-24- albuminuria.
I.E.-cell preparation, Kalin and Kolmer, ami bloat
