INSTRUCTIONS FOR COMPLETING THE GEORGIA ANNUAL HAZARDOUS WASTE REPORT WHICH FORMS TO COMPLETE Form A - Identification, requests Identification information and must be completed by everyone. Form B - Self-Generated Hazardous Waste and its Disposition must be completed by the following: Hazardous Waste Handlers that generated regulated quantities of waste in 1983 and/or had waste in storage at the beginning of 1983. NOTE: I your status is Small Quantity Generator, and no regulated quantity of hazardous waste was on hand in any month of 1983, so indicate by entering zeros on Line 4 of Form B. Form C - Hazardous Waste Received from Georgia Hazardous Waste Handlers and its Disposition must be completed by the following: TSD Facilities that receive hazardous waste from Georgia handlers for treatment, storage, disposal, use, reuse, recycling or reclamation. Form D - Hazardous Waste Received from Out-of-State Hazardous Waste Handlers and its Disposition must be completed by the following: TSD Facilities that receive hazardous waste from out-of-state handlers for treatment, storage, disposal, use, reuse, recyling, or reclamation. It is very possible that you may need to complete a combination of these forms depending on your particular hazardous waste activities. Some examples are as follows: 1. TSD Facilities that generate hazardous waste and receive hazardous waste from other Georgia handlers for treatment, storage, or disposal would -need' to complete Form B for the hazardous waste they generated themselves and Form'C for the hazardous waste they received from other Georgia handlers. 2. TSD Facilities that generate hazardous 'waste and receive hazardous waste from out-of-state handlers for treatment, storage, or disposal would need to complete Form B for the hazardous waste they generated themselves and Form D for the hazardous waste they received from out-of-state handlers. 3. TSD Facilities that receive hazardous waste for treatment, storage, or disposal from Georgia handlers and out-of-State handlers would need to complete both Forms C and D. 4. TSD Facilitiesthat generate their own hazardous waste and receive hazardous waste for treatment, storage, or disposal from Georgiahandlers and out-of- state handlers would need to complete three forms (B, C, and D).
