Due to records retention ARC data is no longer available on the reactor mixture. As a result, it was suggested that ARC runs be conducted to obtain this data. 2. Concerning D-101: re-evaluate the reactive chemical consequences of a scenario involving water in D-101. Determine if the 20$# relief valve is sized properly. 3. Review sodium borohydride storage procedures and determine if the present storage location is adequate for this hazardous flammable solid. 4. The design of the vent header should be evaluated. Determine the possibility of backpressure from high to low pressure systems and if the overall system can handle 2 phase flow. GLYCOL ETELtJ ,' 1. Concerning D-101A: the water scenario (item 2 above) along with sizing of the relief device should be addressee. 2. Consider impIs.anting a positive ID test on the KOH catalyst. 3. Consider implementing safeguards to assure that all instrument work errors are identified and corrected before attempting ** scart-up. An incident ccccrred on May 7, 1990 at Solvents/EDC which was a direct result of this condition.
