INTRODUCTION

Braking systems and braking surfaces are subjected to substan-
mechanical and thermal loads in the interest of the safety of
millions of vehicles in road traffic In particular the highest
requirements are placed on them in terms of strength heat con-
ductivity and heat storage capacity and a possibly constant
friction value 5 In addition to friction value modifiers bind-
ing agents and filling materials conventional braking surfaces
contain asbestos as fibers and exclusively chrysotil called
white asbestos At the present time we assume a yearly consump-
tion of about 10,000 tons of chrysotil for friction surfaces in
the Federal Republic of Germany 16
Asbestos dusts enter the environment during braking processes in the form of emissions In addition they represent a health risk to the vehicle mechanics the extent of which has not yet been determined This is true for repair work of braking systems and couplings where the wear and working dusts can enter the respiratory air
In the following we will discuss four disease cases with a fatal outcome diffused malignant pleuramesothelioma in vehicle workers after being subjected to asbestos dust due to wear and working dusts during brake repair

2. The truck worker born in 1932 first performed braking and
coupling repair during his apprenticeship from 1947 over three
years and for several hours every day The brake systems were
blown off with compressed air for the most part for cleaning from
asbestos containing wear dusts New linings were worked by machines
that is they were ground down and ground off on lathes During
welding work asbestos cardboard softened in water was placed on
to the sheetmetal for cooling and then brushed off in the dry state

After 1950 the patient was a warehouse worker
and warehouse director in a room next to the shop The supply of
braking material containing asbestos was stored loose over several
years without any packaging
At the end of 1981 there were stinging thorax pains and in May of 1982 there was respiration insufficiency The histological diagnosis of a bivalent differentiated and diffuse malignant pleuramesothelioma was determined in the following month with thorascopy After therapeutic treatment the patient died
in March 1983

DISCUSSION
The mesothelioma as a signal tumor for asbestos dust danger
In the general population diffuse malignant mesothelioma are

high asbestos dust danger amounts to 7,21,23 Recently Hartmann 4 discussed the problems of diagnosis guarantees from the point of view of a pathologist in this magazine
Up to the present research has not been capable of identifying other causes for this tumor in human beings except asbestos and comparable mineral fibers The diffuse malignant mesothelioma can be considered as a signaltumor 18,19,23 for asbestos danger Earlier investigations performed in our polyclinic showed that in addition to workers from the asbestos industry especially users of products containing asbestos can suffer mesotheliomas 22
ASBESTOS DUST DANGER FOR THE TRUCK SERVICE INDUSTRY
At the end of 1982 there were over 220,000 workers as truck mechanics trainees or unskilled workers 24 dust danger can be expected both

on a lathe at least for trucks and buses
Asbestos dust dosimetry has become very important for estimated
risks Our dust measurements performed showed that for a sample
duration between 0.5 and 3 minutes during the blowing off of brake
lining heat concentrations of up to 20.106 chrysotil fibers
with a length of > 5 ...mcan occur as shown in Figure 2 when brake
installations are blown off

ASBESTOS INHALATION CONSEQUENCES IN THE TRUCK INDUSTRY
Epidemiological investigations on asbestos inhalation conse-
quences in the truck industry were already performed in the
seventies 10 by the working group of Selikoff In more
than 1/4 of the workers which had worked at least 10 years they
found and described noticeable ray changes in the lungs and

Within our BMFT research project we tried to obtain more reliable results using an expanded investigation program presently with a larger collective In the international literature there has already been published reports about mesothelioma in conjunction with friction lining work For example Otto 13 published the case of a year old locksmith who after having having installed brake linings over many years became ill with a pleura mesothelioma Another illness case was described by Langer 9 A year old man who was a truck mechanic since his 19th year and especially
worked on brake systems died because of a diffuse pleuramesothelioma

SPECIAL FEATURES OF OUR OWN CASE STUDIES
In the four cases which we observed with diffuse malignant
pleuramesothelioma occupational contact occurred already at ages between 14 to 18 years x - s = 15.3 - 1.9 years i.e. when they
were young

especially high fiber concentrations The first patient code 4

Laws Mesothelioma caused by excesses of the rib pleura and the
peritonieum which is the law for all physicians First commun-
ication was established with the associated health insurance company
In the meantime 3 of the 4 mesothelioma cases have been recognized
as worthy of damages in terms of occupational cancer cases
However this did not occur during the life of the patient In

the 4th case the investigations by the authorities have not yet
been concluded
Our studies have confirmed that diffuse
malignant mesothelioma . occurs among truck workers A more exact risk estimation has not been possible yet
from
selection effects and from the suspected false numbers
We have to emphasize again that the low fiber doses as

measured today are not representative for the
work place situations

However we have
to ask ourselves whether for the high fiber concentrations caused
by grinding friction surfaces shown in Figure 1 there has not been a
substantial increase of asbestos danger even if there were suction
devices combined with other working methods which were practiced
more often in the past

disease appeared on the average 32 years after the first ex-

tient within merely 2 to 9 months after

1. Der 1937 geborene Patient begann im Alter von 14 Jahren eine Lehre als Schlosser Nach Lehrabschlu^ war er bis 1978 in verschiedenen Werkst^/ttent^/tig U.a. f^...hrteer Wartungs- und Reparaturarbeiten an Bremssystemen in PKW- und Werk-
st^/tten aus Insbesondere in den 50er und 60er Jahren erfolgten
die dabei notwendigen Arbeiten ohne Absaugung der asbesthal-
tigen Abrieb- und Bearbeitungsstaube
Im Mai 1982 traten Husten und zunehmende Brustschmerzen
auf Ab September 1982 entwickelte sich zus^/tzlicheine Belastungsdyspnoe Eine Thorakotomie f^...hrtezur histologischen Diagnose eines epithelialen diffusen malignen Pleuramesothelioms Die Therapie mu^ tesich auf eine Teilpleurektomie mit Teilresektion eines Lungenlappens beschranken 3 Monate spater verstarb der Patient an den Folgen des Tumorleidens

Der 1932 geborene Handwerker hatte zun^/chstw^/hrend der Lehre von 1947 an 3 Jahre lang mehrere Stunden am Tag Brems- und Kupplungsreparaturen vorgenommen Die Bremssysteme wurden zur Reinigung von asbesthaltigen Abriebst^/uben ^...berwiegendmit Pre^ luftausgeblasen Neue Belage wurden maschinell bearbeitet d.h. abgeschliffen oder ^...berdrehtBei Schwei^ arbeitenwurde in Wasser eingeweichte Asbestpappe zum K^...hlenauf das Blech gelegt und hinterher trocken abgeb^...rstetEine Umluftanlage soll auftretenden Staub gleichm^/^ ig in der Halle verteilt haben Ab 1950 war der Patient als Lagerist bzw. Lagerleiter in einem an die Werkstatt grenzenden Raum t^/tigDer Lagerbestand an asbesthaltigen Bremsbel^/genwurde lange Jahre ohne Verpackung lose im Regal gehalten
Ende 1981 kam es zu stechenden Thoraxschmerzen im Mai 1982 zu Atemnot Die histologische Diagnose eines bivalent differenzierten diffusen malignen Pleuramesothelioms wurde im Folgemonat thorakoskopisch gestellt Nach chemotherapeutischer Behandlung trat der Tod im M^/rz1983 ein

Alter von 51 Jahren schied der Patient durch Suizid aus dem Leben
