Jahn INTRODUCTION tial Braking systems and braking surfaces are subjected to substan- mechanical and thermal loads in the interest of the safety of millions of vehicles in road traffic In particular the highest requirements are placed on them in terms of strength heat con- ductivity and heat storage capacity and a possibly constant friction value 5 In addition to friction value modifiers bind- ing agents and filling materials conventional braking surfaces contain asbestos as fibers and exclusively chrysotil called white asbestos At the present time we assume a yearly consump- tion of about 10,000 tons of chrysotil for friction surfaces in the Federal Republic of Germany 16 Asbestos dusts enter the environment during braking processes in the form of emissions In addition they represent a health risk to the vehicle mechanics the extent of which has not yet been determined This is true for repair work of braking systems and couplings where the wear and working dusts can enter the respiratory air In the following we will discuss four disease cases with a fatal outcome diffused malignant pleuramesothelioma in vehicle workers after being subjected to asbestos dust due to wear and working dusts during brake repair Institut aned Polyclinic for Occupational and Social Medicine of the Liebig University Giesen Director Prof. Dr. Joachim Woitwitz With financial support by the West German Federal Ministry for Research and Technology Bonn Project no 01 VD 2900 CASE DESCRIPTION 1 The locksmith at patient the age born in 1937 of 14 years started his apprenticeship as a After concluding apprenticeship in 1978 he worked in various shops He had to perform maintenance and repair work on brake systems in automobiles and truck shops In particular during the 50's and 60's the necessary work was performed without sucking off the worn off and working dusts which is now required In May 1982 coughing occurred and increasing breast pain After September 1982 a load dyspnoea A thoracotomia led to histological diagnosis of epithelia and diffuse malignant pleura- mesothelioma The tomia with partial therapy had to resection of a be restricted to partial lung flap Three months pleurec- later the patient died as the consequence of a tumor 2. The patient was born in 1931 and worked for over 18 years beginning For 10-20 on his 14th birthday as a mechanic in various hours per week he repaired brake systems with truck shops the release of wear and working dusts containing asbestos and In July 1982 the disease started with respiratory insufficiency breast pain In October the histological diagnosis of an epithelian differentiated diffuse and malignant pleuramesothelioma was detected from a histological diagnosis The treatment was restricted to systematic and analge tic measures First up to 12 morphium injections were necessary every day At the age of 51 years the patient died by suicide 4. Age at beginning of asbestos dust danger Time duration of asbestos dust danger duration since the first asbestos dust danger latency time and age at the time of death for four vehicle workers with diffused malignant pleuramesothelioma age at the age at patent beginning of asbestos time duration since of since first the of time code dust danger danger years death 1 14 27 31 45 2 15 35 35 50 3 14 18 17 51 4 18 11 26 44 DISCUSSION The mesothelioma as a signal tumor for asbestos dust danger In the general population diffuse malignant mesothelioma are tumors on the which other are very rare Numerous hand shown a substantial epidemiological studies have increased expectation fre- quency for certain groups subject to asbestos dust danger In the general population there are between 1-8 mesothelioma cases per one million inhabitants That is there is one death case per 1000-10,000 deathisn the general population The fraction of mesothelioma death cases in groups with a high asbestos dust danger amounts to 7,21,23 Recently Hartmann 4 discussed the problems of diagnosis guarantees from the point of view of a pathologist in this magazine Up to the present research has not been capable of identifying other causes for this tumor in human beings except asbestos and comparable mineral fibers The diffuse malignant mesothelioma can be considered as a signaltumor 18,19,23 for asbestos danger Earlier investigations performed in our polyclinic showed that in addition to workers from the asbestos industry especially users of products containing asbestos can suffer mesotheliomas 22 ASBESTOS DUST DANGER FOR THE TRUCK SERVICE INDUSTRY At the end of 1982 there were over 220,000 workers as truck mechanics trainees or unskilled workers 24 dust danger can be expected both employed Asbestos - when working on new linings and - when removing the worn off dust The reworking of automobile drum brake linings usually is done by grinding down using brake grinding machines or by hand Figure 1 On the other hand truck and bus brake linings are mainly processed with brake lining rotating machines The removal of the working dust is done by with small brushes as well as blowing off brushing off or with compressed removal air Figure 2 Our own field investigations were performed within the framework of a private project supported by the West German Ministry for Research and Technology BMFT for estimating the health risks in truck shops caused by asbestos In several other investigations 2,5,17 there was a substantial reduction in the working processes which were subject to asbestos dust danger 15 364 This can be attributed to the use of tested parts in use today In this way work processes on brake lining no longer are needed as before such as the cutting braking of edges and rivet hole drilling Before it was natural to perform lining grinding as shown in Figure 1. Today the brake linings are worked on a lathe at least for trucks and buses Asbestos dust dosimetry has become very important for estimated risks Our dust measurements performed showed that for a sample duration between 0.5 and 3 minutes during the blowing off of brake lining heat concentrations of up to 20.106 chrysotil fibers with a length of > 5 ...mcan occur as shown in Figure 2 when brake installations are blown off The asbestos dust danger for 5 Figure 1 Grinding of a truck brake coating containing asbestos on a grinder with sucking off of the working dust During grinding of a large area layer asbestos fiber con- centrations occur which can be calculated at 5.106 chrysotil fibers with the range of > 5 ...m from a working process which includes dust measurement over 5 mins During one breath accordingly a volume of 1-2 liters or about 5000-10,000 chrysotil fibers with L > 5 ...um were inhaled For a respiration air consumption of 20 liters per minute results in a total of about 500,000 chrysotil fibers with L > 5 ...m for a grinding process with 4 brake coverings of one axle which extends over 5 mins Since in spite of this sucking off of the asbestos dust the asbestos dust danger can exceed the technical directive concentration TRK brake coverings of buses and trucks in general today are no longer ground off but are turned on the lathe the characteristic work processes including cleaning of brakes and the working of brake linings is best described by the average of the asbestos fiber doses determined from light microscopy This is a product of the measured fiber concentration and the time duration of sampling 8 For automobiles during the blowing out process and during grinding the product of the sampling duration and fiber concentration is observed to be about 4.106 fibers minutes corresponding to a 4 minute concentration of 106 fibers For trucks and buses this product is about 6.106 fiber minutes for fiber 8.106 blowing out and 8.106 minutes for lathe working It is 14-106 fiber mm for grinding processes These fiber doses are evaluated to be small compared with the asbestos is the maximum allowable amount when maintaining dust danger which the technical directive concentration of 106 fiber per work shift at 480 6 minutes or 480.106 fibers minutes at the present time Electronmicroscope analyses sometimes give higher asbestos fiber dose values Especially in all chrysotil fibers are observed dry dust more than 99 of which have a length of < 5 m Such fibers cannot be detected by usual light microscopic detection methods there is a danger 17 because of due to asbestos insufficient diameter containing dusts which Therefore can be breathed in both for processing of new linings and also for the unprotected removal of pressed air Recently the friction dust refined analytical for example methods using com- energy dispersing ray microscopic have allowed one to detect chrysotil fibers in the friction dust 16,17 . analysis electron refraction also for the majority of We can no longer assume a complete mechanical or fraction of the linings legend of Figure 2 thermal conversion of the for example in forsterit asbestos fiber 5 See the ASBESTOS INHALATION CONSEQUENCES IN THE TRUCK INDUSTRY Epidemiological investigations on asbestos inhalation conse- quences in the truck industry were already performed in the seventies 10 by the working group of Selikoff In more than 1/4 of the workers which had worked at least 10 years they found and described noticeable ray changes in the lungs and pleura et al i.S. the 1 found indication of only one case a starting asbestosis Boillat of a beginning asbestosis from ray analysis on a sample of 30 mechanics in the automobile brake service A cross section study performed at the beginning of the 70's in the West German area as a pilot study found that among 160 truck mechanics which had repaired brake installations for more than 10 years there were two cases for which asbestosis had to be assumed to an extent accident insurance policy which is important in terms of legal However in both cases there was excessive importance attached to chronic bronchitis with compression 7 Figure 2. The following has to be stated in terms of occupational protection for young workers Peto 14 determined that the mesothelioma incidence for given asbestos fiber danger increases as a function of time since the first danger that is the risk duration with a power of on whether the danger is over a short time 3 or that 4. is This depends within a few years or existed continuously Peto calculated for as an example a school contaminated with sprayed asbestos that asbestos dust danger between the 12th and 18th can lead to one mesothelioma case per 100,000 for a concentration of only a few thousand fibers . For higher concentrations the number of mesothelioma increases linearly This fact should lead doctors to carefully observe and apply the directives for the young worker protection laws for activities where there is danger by cancer producing materials The duration of occupational asbestos dust danger of our patients was between 11 and 35 years X - s = 22.8 - 10.5 years The working of the friction layers at the time was done in a typical fashion The working dust in general was removed with compressed air without sucking it away There were especially high fiber concentrations The first patient code 4 a truck electrician was exposed because of adjacent work places a called bystander exposure which took place over 11 years However 1976 Rohl et al 17 indicated that several minutes after blowing off a brake drum with compressed air one could measure increased asbestos concentrations with a light microscope for distances up to 25 meters From an occupational medicine point of view we have to note that in individual cases breathing in of relatively small dust doses can produce lioma already the a mesothe- year The mesothelioma cases which we have presented in four truck workers occurred after a latency period of 26-37 years X - s = 32.3 - 49 years In larger comparable group of patients the median value of latency time was 30 years We talk of the called 30 year rule 22 Independent of therapeutic measures the disease led to death in all cases within a short time see Table 2 In the literature the survival times from symptoms was given as 10 months for one half of the the beginning of patients 6 10 The occupational disease methods in 3 patients started by the notice in the BK News of no 4105 of the Occupational Disease Laws Mesothelioma caused by excesses of the rib pleura and the peritonieum which is the law for all physicians First commun- ication was established with the associated health insurance company In the meantime 3 of the 4 mesothelioma cases have been recognized as worthy of damages in terms of occupational cancer cases However this did not occur during the life of the patient In 366 the 4th case the investigations by the authorities have not yet been concluded Our studies have confirmed that diffuse malignant mesothelioma . occurs among truck workers A more exact risk estimation has not been possible yet from selection effects and from the suspected false numbers We have to emphasize again that the low fiber doses as _ measured today are not representative for the work place situations 20 , 30 or 40 years ago However we have to ask ourselves whether for the high fiber concentrations caused by grinding friction surfaces shown in Figure 1 there has not been a substantial increase of asbestos danger even if there were suction devices combined with other working methods which were practiced more often in the past REFERENCES 1 Boillat M. Der 1932 geborene Handwerker hatte zun^/chstw^/hrend der Lehre von 1947 an 3 Jahre lang mehrere Stunden am Tag Brems- und Kupplungsreparaturen vorgenommen Die Bremssysteme wurden zur Reinigung von asbesthaltigen Abriebst^/uben ^...berwiegendmit Pre^ luftausgeblasen Neue Belage wurden maschinell bearbeitet d.h. abgeschliffen oder ^...berdrehtBei Schwei^ arbeitenwurde in Wasser eingeweichte Asbestpappe zum K^...hlenauf das Blech gelegt und hinterher trocken abgeb^...rstetEine Umluftanlage soll auftretenden Staub gleichm^/^ ig in der Halle verteilt haben Ab 1950 war der Patient als Lagerist bzw. Lagerleiter in einem an die Werkstatt grenzenden Raum t^/tigDer Lagerbestand an asbesthaltigen Bremsbel^/genwurde lange Jahre ohne Verpackung lose im Regal gehalten Ende 1981 kam es zu stechenden Thoraxschmerzen im Mai 1982 zu Atemnot Die histologische Diagnose eines bivalent differenzierten diffusen malignen Pleuramesothelioms wurde im Folgemonat thorakoskopisch gestellt Nach chemotherapeutischer Behandlung trat der Tod im M^/rz1983 ein 3. Der 1931 geborene Patient war seit seinem 14.
