Environmental Protection Agency Region 4 Air Enforcement Branch 61 Forsyth Street, SW Atlanta, Georgia 30303 INSPECTION REPORT Facility Inspected 45 South Main Street Ellijay, Georgia 30540 Inspection Date: May 19, 2022 Facility Programmatic ID 3601431265 LAT: 34.6943 LON: -84.4828 Inspection Type Clean Air Act (CAA) National Emission Standards for Hazardous Air Pollutants (NESHAP) - Asbestos Inspectors Pamela Storm Tony Spann Introduction An inspection pursuant to Section 114 of the CAA was conducted on May 19, 2022, at 45 South Main Street in Ellijay, Georgia 30540 (the facility). The facility is owned by J. Chance Investment (P.O. Box 95, Blue Ridge, Georgia 30513). The unannounced inspection was conducted by Pamela Storm who possesses EPA inspector credentials to conduct CAA NESHAP asbestos inspections and Tony Spann. Mr. Spann also possesses EPA inspector credentials and was participating in this inspection to meet training requirements to conduct CAA NESHAP asbestos inspections. Permit #740 was issued on January 12, 2022, from the City of Ellijay for repairs to the facility. This permit listed Cox Roofing and J&S Construction as the roofing contractor and the demolition contractor, respectively. The inspectors obtained information regarding this renovation from the City of Ellijay on May 19, 2022, prior to this inspection. The purpose of the inspection was to determine compliance with the CAA NESHAP Asbestos Standard and the regulations promulgated at 40 CFR Part 61, Subpart M, for the renovation and demolition activities conducted pursuant to this permit. The EPA Region 4 CAA NESHAP asbestos program targets facilities for inspection located in the State of Georgia based on tips/complaints; permits issued on a local level; and demolition and/or renovation/abatement notices sent to the State. This inspection was conducted based on the previously mentioned information from the City of Ellijay. Facility Information The facility was not occupied at the time of this inspection. Renovations were ongoing. Inspection Inspection At the time of this inspection, the facility was unoccupied and was undergoing renovations. The facility is located at 45 South Main Street in Ellijay, Georgia at the intersection with Spring Street. A sign in the parking lot of the CAA Asbestos Inspection Report: Inspection Date: 45 S. A door facing Church Street at the rear of the facility has a sign with haircutting scissors that reads, "Studio Entrance" indicating that a portion of the facility may have previously been used as a salon. The inside of the facility was gutted. Interior walls had been removed. The ceramic flooring was removed from half of the flooring space. Ceiling panels appeared damaged. Electrical boxes had been removed from the walls. A staircase inside remained standing. Observations were made through windows from the outside of the facility. Conclusion The inspectors will follow up with the facility owners and operators, as deemed appropriate, to obtain information and necessary paperwork related to these renovation and demolition activities. This information will include, but not necessarily be limited to, copies of asbestos inspection reports and renovation and demolition notifications for the facility.
