G. Trafton SHAC D. E. Kaldenberg Paul Siems Bubba Bean The Fourth Quarter Safety and Housekeeping Inspection for Dept. 48/56 was conducted on 1/24/84 by Paul Siems, Bubba Bean and Bob Hammann. Overall the unit was in good condition. Please respond to the following recommendations in writing to J. G. Trafton with carbon copy to Steve Moncla by 2/21/84. Personal Knowledge of Safety Most records are in excellent condition. Need to set up monthly safety meetings and unit craft meetings and keep minutes of these meetings. Copies of the minutes should be sent to Loss Prevention and Safety. Personal Safety Equipment 1. There is need to establish the hearing conservation program in the department. Areas where hearing protection is required should be marked and enforcement started. (See attached memo.) 2. Five minute escape mask missing from top deck of the hydrogenators. 3. Portable eye wash in motor control center had little water in it. Condition of Safety Equipment 1. Signs missing on water and nitrogen stations top deck of hydrogenation. 2. Safety chain missing on chain operated valve by 48-604. Safety Hazards 1. Cable broke on air hoist east end of hydrogenators. 2. Need better cleanup around the area south and west of maintenance shop as it is extremely hazardous due to the clutter created by construction. 3. Chain is broken on valve top floor of Dept. 56. SC 000611 LAM001809 Vernon Fields Page 2. Housekeeping 1. Scaffold storage area south of Dept. 48 needs cleanup. 2. New addition north end of 3rd floor in Dept. 56 very cluttered. 3.
