Sharp - PER The 2nd Quarter Safety and Housekeeping Inspection for Department 15/16 was conducted by the inspection team of Jim McPhail, Mike Burkhalter, Bill Recknor and B. J. Sanders on April 15. The department was given an "outstanding" rating. The following items were noted: Safety 1. AVIP was up-to-date. Department 15 100Z, Department 16 99Z with 1Z exception properly handled. 2. Interlock bypass procedure well documented, up-to-date and rigor ously followed. Keep up the good work on these items! 3. Contractors using manlift on north end of Department 16 without signs posted. They were questioned about manlift operator training. They have not been trained by Monsanto; one was trained at Carbide, the other has no formal training. 4. Wire hangar temporary support on 4160v line west side of 15C1-1. 5. Fire extinguisher missing on south side of Department 16 (No. 16-30). 6. No inspection tag on ladder at base of 16D3. Ladder belongs to Department 13/14. 7. Bleed valves on several 4" fire monitors needs to be replaced. Housekeeping 1. Excess oil on pad around 16P66. 2. 16P6 suction line needs support. 3. Steam inlet to 16J8-2 needs insulation repair. SC 000335 LA M001532 *) 4. Leak in FW supply to Department 15. 5. Blind and bolts laying under 15T24. 6. No. 6 nitrogen station needs repair. 7. Loose hose needs storing In NSI area. 8. Empty drums on north side of 16E8-1. 9. Water line needs replacing above 15T23. One item needs special mention. Mike Burkhalter alertly noticed a potentially serious condition at several fire monitors where gate valves have been replaced with long handle plug valves. When opened, the plug valve handle prevents complete 360 movement of the monitor head. This condition could exist many places throughout the plant and should be checked out by the Fire Department.
