Comments on Statistics We experienced two Recordable Injuries in October. An Instrument employee fractured his left little finger when it was caught between a regulator and a piece of pipe. A machinist received a laceration, requiring several stitches, when struck by a "lathe dog" while drilling a piece of plastic pipe. There were twenty-three Total Injuries. Over half of the injuries last month were to the hand and arm which indicates that we need to place special emphasis in this area. It is recommended that the topics of "Material Handling", "Tool Safety" and "Equipment Operation" be addressed in our departmental safety meetings, day-to-day activities and safety surveys. It would also appear timely to address the accident type Slips and Falls due to the upcoming cold and wet weather. This will help increase each employee's safety awareness and improve plant-wide safety performance. C. Units and Groups with Rising Trendlines Carpenters, Power 2, Truckers and Laborers all having rising trend lines since early this year. Sharply rising, but still within the control limits, are the electricians. It is noteworthy that both operators and all maintenance show rising trend lines since April. D. Contractor Safety--C.M. McCullough Total contractor safety results are improving. However, total injury rates of Capital Projects contractors are slightly higher than last year. Monthly summaries and year-to-date figures are attached. E. CEP Project Safety--Bill Napier Bill reported that they now had stickers that they used to indicate when contract workers had had their safety orientation. Bill also reported on the cable tray incident and a broken toe incident. Following a discussion of the scaffold incident which had ended in a court award, the Plant Manager asked Arnold Glass to review this incident and the problems in investigation of accidents and injuries that occur to contract personnel with the St. Louis Attorney, Bill Blase (this has been done). J SC 003359 LAM004373 PMSB Minutes of Meeting of November 19, 1980 Page 3 II. REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC)--G.W. Daues A. EPP Procedure Recently, Tom Ryan and Arnold Glass described the duties and proce dures to be used in the EPP Control Center. This Information was distributed to all exempt employees. SAC recommends it be incor porated into the Emergency Procedure Plan Section I. Consideration should be given to distributing to non-exempt and perhaps hourly employees. The PMSB agreed and requested that we train Night Superintendents/Shift Supervisors on how to operate the EPP console. B. EPP Drill Plans Arnold Glass reviewed the outline of the EPP drill planned for next month. SAC concurs that the plan is very good and much needed. C. Employee Training A problem with updating safety training was discussed. SAC feels that several training programs used in the past are still effective and should be updated, if necessary, and distributed by LP&S to line supervision for use in monthly meetings. Also, certain programs could be used for the S-T-A-R-T Program. LP&S will distribute training materials on special programs "through the line". D. Breathing Air System It was brought to the attention of SAC that problems continue to occur with our plant breathing air system. SAC recommends that Engi neering should give higher priority to the study of this system according to the guidelines issued by G.L. Tromblee last month. This has been done. Target for AR: to be sent to St. Louis by December 15, 1980. E. Telephone Problems Emergency and effective use of telephones, problems with our telephone system for emergency reporting and false alarms were discussed. SAC recommends that this subject should be studied by the Office Safety Committee to obtain recommendations for the PMSB. M1SD will determine needs and recommend actions for false alarms. CSC will review other needs and respond to LP&S (Bob Hammann) for presen tation/review with SHAC/PMSB. F. Fire, Entry and Special Permits Revisions to P-0017 on fire, entry and special permits, as suggested by Bob Hammann, were reviewed and after minor changes will be recommended to the PMSB. Sc 03360 LAM004374 PMSB Minutes of Meeting of November 19, 1980 Page 4 G. 1980 Annual Inspection Recommendation The inspection team requested we review the current practice in the Texas city plant with regard to work permits. SAC's recommendation that the use of all work permits be on a unit-option basis was accepted by the PMSB and approved on July 23, 1980. Reconsideration of this recommendation was given and it is the concensus of SAC mem bers that the PMSB decsion of July 23, 1980 stand. PMSB agreed. H. Face Shields A survey by LP&S of the use of face shields in the TC plant was reviewed by SAC and the recommendations from this survey were approved by SAC with minor modifications. LP&S will review these recommendations with line supervision prior to the November PMSB meeting. I. Superhazardous Materials At the October PMSB meeting, the Board approved a 1979 recommendation that intradepartmental identification and safety markings of equip ment and piping containing superhazardous materials be determined by those in each unit and not be an across-plant procedure. A Pprocedure on this subject which was never issued is still being reviewed by SAC. III. REPORT OF THE SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC)--C.M. Cruse A. 1980 Safety and Housekeeping Inspection--Refer to memo MCI Annual Inspection 1980, from J.A.G. to PMSB and SHAC. Target dates for completion to be established--J.A. Glass The memorandum regarding the MCI Annual Inspection was passed out and will be reviewed at the next meeting. One item in this list, being worked on by SHAC, concerned the recom mendation about continuation of the ladder inspection program. SHAC recommends a return to "inspect-before-use" and monthly inspections with heavy emphasis on individual responsibility. This will be reviewed in some detail at the next SAC meeting. Another item being considered by SHAC was emergency tie-down material. This item came up as a result of the eye injury which occurred from the use of wire in tying down loose scaffold boards. SHAC recommends no change in the present material being used. The PMSB recommended that SHAC get suggestions from the carpenter craft for better techniques in how to use the wire safely. Overall empha sis in avoiding accidents and injuries in the use of wire will require education of the persons using the material. Cruse SHAC reports that at the time of the meeting, the Vessel Inspection Program was 95% on target and expects to be higher, possibly up to 9 7%, by year end. C. Status of S-T-A-R-T Safety Program--C.M. Cruse The second session is well along. All Maintenance is now complete. The third session will begin early in December. D. Hourly Safety Committee Items--C.M. Cruse Individual bicycles have been issued to members of the Hourly Safety Committee. The Hourly Safety Committee is actively working on a list of items on their agenda. They expect to submit a report of the hierarchy of lock-out at the next meeting of this group. IV.
