Section I - INTRODUCTION
PURPOSE OF THE INSPECTION
EPA Region 6 Inspector, Matthew Allen, arrived at the H.R. Collier - Osage Happy Hollow Lease (EPA ID No. OKU002220) at 10:20 AM on 08/04/2025 for a follow-up inspection in response to a spill inspection conducted 7/30/2025 (NRC# 1438334). The inspection was conducted by the authority of the Clean Water Act (CWA) to determine compliance with Environmental Protection Agency (EPA) regulations. There was no company representative present during the inspection. There was no other federal, state, or local agency representative present at the time of the inspection.
FACILITY DESCRIPTION
EPA conducted a follow-up inspection at the H.R. Collier, Osage Happy Hollow lease where a ruptured poly flowline released a company estimated five (5) barrels of oil and one (1) barrel of saltwater to the land surface. Spill impacts migrated to a nearby unnamed tributary of Little Hominy Creek and travelled generally south, ultimately impacting Little Hominy Creek.
Section II - OBSERVATIONS
I observed the first pool of impacted water along the unnamed tributary of Little Hominy Creek. A TDS field reading of 369.7 mg/L was observed. See photo No. 1 and 2. For additional GPS location information and photo metadata, please see Appendix 1 (photo log) attached to this report.
I observed a TDS field reading of 399.4 mg/L at the mouth of the impacted tributary, immediately upstream from the confluence with Little Hominy Creek. At the same location, I observed an oil absorbent boom installed across the impacted tributary. At the time of the inspection, the boom was collecting a small amount of oil. See photo No. 3 and 4.
I observed the impacted section of Little Hominy Creek. At the time of the inspection, there was no oil, sheen, oil-stained banks, or oil-stained vegetation/debris present in Little Hominy Creek. See photo No.5.
Section III - AREAS OF CONCERN
The primary area of concern was the small amount of oil present at the oil absorbent boom near the confluence of the impacted tributary and Little Hominy Creek.
Section IV - FOLLOW UP
EPA inspector will conduct follow-up calls, field meetings and inspections to ensure the impacted area is returned to "pre-spill" condition.
