Class I and II Well Inspection Form US Environmental Protection Agency - Region 4 Underground Injection Control Program 61 Forsyth Street SW, MC 9T25, Atlanta, Georgia, 30303 Phone: (404)562-9424 EPA Well ID#: Year-Month-Day: 2 0 2 1 - 1 1 EPA Permit # (or RA): Page 2 - 1 8 Of 2 (Cont) Laurel Field Unit (LFU) 36-13 #1 36-13-1 Class II Well/SPCC Observations: The operators informed the inspection team that the drilling field has two 16,800 gallon aboveground storage tanks (AST)s on site that are approximately 40 % full with produced fluids and four 8,820 gallon ASTs on site that are approximately 60 % full with produced fluids. The inspection team did not observe any other ASTs in this drilling field. The P.E. certified SPCC Plan for this drilling field stated that soil should be used for secondary containment for the six ASTs. The tank farm uses soil (permeable material) as a secondary containment for the six aforementioned ASTs. The six ASTs were badly rusted and looked like they were poorly maintained. Team's observations/comments: We Inspected 8 wells that day, with a total of 9 people: 4 from Petro Harvester, 2 from MSOGB and 3 from EPA. All inspected wells were located in areas that were fenced and gated. All the pumping units and injection pumps were in good condition and operated almost silently. The well locations seemed to be kept clean and organized. The focus of this inspection was well 36-13-1 which was TA with 0 psi on Tubing and 0 psi on Annulus. Also, the well was chained and locked in the closed position.
