The cause of their symptoms is not known, but some ingred ent of the polyvinyl chloride film is suspected. Report of Cases Cass l.-A 40-ycar-old woman who had worked as s supermarket meat wrapper far ten yean without respiratory diffi culty was lint wen in 1972. Believing that her res piratory problem was related to the plastic fumes, she quit her job. In the ensuing two years, she has developed nonscasunal wheezing, shortness of breath, and cough ing, punctuated by upper respiratory in The time required for development of symptoms was brief (30 minutes) in patients - and 3 and was delayed (five hours) in patient 1. In patients, symptoms eventually occurred inde pendently of fume exposure, and she now has chronic dyspnea and pro was introduced to wrap the meat in a plas fections. On returning to meat wrapping, she developed shortness of breath and wheezing within 10 minutes. The dyspnea became so dis turbing that she came to hospital emer gency room. months nf the year. The patient smoked one pack of cigarettes per day for nine yean, but she slopped smoking when her present illness began. History showed a contact dermatitis but no previous history of hay fever, rhinitis, or asthma. Results of physical examination (five hours after leaving work) were normal. Complete blood cell count, erythrocyte sedimentation rate, routine blood determi nation, latex fixation, VDltL test for syph ilis, and chest x-ray film were within nor mal limits or negative. Skin tests with common allergens were negative. Pulmo- chest, which showed scattered wheezes an expiration and persistent bibasilar crepi tant rales. * Laboratory teats including a complete blood cell mint, erythrocyte sedimentation rate, VDUI, test for syphilis, and routine blood chemistries were w*ithin normal lim its. An x-ray film of the chest showed mild overinfiatioa, with apical capping of both upper lobes. Histoplasmin and inter- history of atopy. Wc do not know the frequency of this syndrome, but wc suspect it is not rare. The first patient told us about the second patient. Wc V__ maximal expiratory (low rate that im proved with bronciiodilation. The patient was given liruncliodilators orally to take . prophylactieally. Using this medication, she has been able to wrap meat inter mittently without any respiratory symp toms. She is asymptomatic after w-orlt and on weekends, requiring no medications at those times. Case 3.--A S-l-vear-old woman Itad a three-yenr history of intermittent wheez ing and coughing. Her illness began six negative. A ccxxidieidiii skin test gave 10 mm of induration at 4S hours. Inlradrrmal tests to common allergens were negative. Pulmonary function studies showed a de creased vital capacity and an increased functional residual capacity. The forced vi tal capacity responded to bronehodilation. Nitrogen washout was increased to 4.6% retention after seven minutes of oxygen breathing, with a normal of <2%. The manufacturer of the polyvinyl chloride film used by these meat wrappers has informed us that be cause of previous rc|iorts from several cities of respiratory problems associ ated with polyvinyl chloride fumes produced in the meal wrapping pro years after she started working in a super These three ease reports strongly cess, an investigation nf the matter market as a meat wrapper. Many people working in the area complained of the fumes produced when the film was cut. Thirty suggest that a causal relationship ex ists between the inhalation of plastic fumes and respiratory symptoms. In each instance, the plastic film was polyvinyl chloride made by the same manufacturer. Szende et al` described a 31year-oid factory employee rko shov eled polyvinyl chloride dust. His chest roentgenogram showed manyminute nodulcs. alonp with an inter stitial fibrotie pattern. Lung biopsy specimens showed a foreign body sur rounded by a granuloma, with strik ing cosinophilix. The foreign body eould be dissolved with a polyvinyl chloride solvent, evelohexanol. The problem posed by our patients is whether fumes generated from healed polyvinyl chloride arc toxic Cornish and Abar* pyrolized polyvinyl chloride polymers in a stream of air by gradually raising the temperature from ambient to 600 C. Exposure to this air stream supplemented with oxygen produced pulmonary edema and interstitial edema in rats. Some animals also showed focal bronchial and intra-alvcolar hemorrhage. It is doubtful, however, if this level or ex posure was rvrr experienced by our patients. Our patients resemble somewhat those individuals, first described by Harris,' who experienced respiratory difficulty when exjiosed to heated Tef lon. This syndrome has been called polymer fume fever. The symptoms include attacks of tightness of the chest, difficulty in breathing, some times with a dry, irritating cough, and fever licginning several hours af ter exposure to the hrat-dcgradalion products of polytclranuoruclhylcne (Teflon). Recovery is rapid and usu ally rimplclc. The patients who de velop this syndrome work in plants where )mlytelralluoroc-thylcne is pro cessed, and smoke cigarette*. The' plastic adheres to the cigarclt- and is given olT when heated to > 300 C as a very fine particulate sublimate. The syndrome is thought to lie due to the inhalation of this sublimate.*"* Our patients differed from those with polymer fume fever by not expe riencing chills, fever, or tore throat. This example of a polymcr-fume-relatcd syndrome, coupled with the evi dence that polyvinyl chloride may have some toxic potential strengthen our suspicion that the patients' symp toms are caused by exposure to the polyvinyl chloride fumes. Cornish and Abar* have reported that carbon monoxide and hydrochlo ric acid are the products of major tox icologic importance from the combus tion of vinyl chloride polymer. O'Mara et al,* in describing the combustion profilp of vinyl chloride monomer, noted phosgene as well as hydrochlo ric arid and carbon monoxide. Hydro chloric acid and phosgene are both bronchial irritants that could conceiv ably cause the patients' symptoms. Bei Ubcrprufung dcr Lungenfunktion (Anamnese, Ronrgenuntersuchung, Ruhespirotnetiie, Ganzkorperplethysmographic, Blutgasanalysc) zeigten sid> bei neun Patientcn Hinweise auf vonviegend restpktive Veranderungsn, bei adit Patientcn fcestand eine Panialinsuffizienz. Ob diese Storungen darauf zuruckzuffihren sind, daG sidi an der Lunge ahnlidie Vcrandcrungen abspielen vie am kollagenen und dastischen Bindegewebe der Haut, also eine beginnende diffuse Lungenfibrose zu grunde liegt, mfissen wa'tere Untersuchungen zeigen. Der auslosende Faktor Es handelt sidi um ein beruflidi bedingtes Krankheitsbild, dessen fuhrende Symptome (nach der Haufigkeit geordnet) Thrombopenie, Splenomegalie, ein toxischer Leberscbaden, Vemilationsstdrungen, Durchblutungsstorungen, i Iaut- und Knochenveranderungen sind. Dieser Symptomkomplex isc keinem bishcr bekannten Syn drom zuzuordnen. Atiologie und Pathogenese sind ungeklart. Gelingt es, durdi Vergleich des bishcr aus der Literatur Bekannten mit unseren Ergebr.issen einen gewissen Einblick in Atiologie und Pathogenese zu gewinnen? Was ist als auslbsender Faktor denkbar? Flussiges Vinyldilorid polymerisiert nach Zugabe von Wasser und zahlreidien, nach den Eigcnsdiaften des gewumchten Kunststoffes unterschiediichcn Additiven bei glcichzcitiger Druck- und Tcmperatursteigerung zu Polyvinyluhlorid. UnabhangigvondcrArtderZusatzstofte wurden in verschiedenen Landern bei Autoklavenreinigern ein Raynaud-Syndrom und Akroosteoiysen festgestellt. Diese Symptome schienen so kenmcichnend, daG sie in den Ietzten Jahrcn zur Krankheitsbezeidinung berufsbedingte Akroosteolyse* fiihnen und zur Diagnosestellung gefordert wurden (23). s ist nicht anzunehmen, daG die nadi Hcrstellungs'and und -verfahren verschiedeuen Additive eine wesenilidie Rolle spielen.wcnn auch sdilosscn werden kdnncn. Damit riickt das Vinyldilorid, die mcngenmaCig am stiirksten vertretene Substanz, in den Mittelpunkt des Inieresses. Das Monomer Vinylchlorid svird zunachst iiber ein gesdilossenes System in die Autoklaven gepumpt. Mdglidikeiten des Gasaustrittes -- und damit der Inhalation -- bestehen jcdcch innerhalb dc\ l'roduktiousprozcsses an mehreren Stcllen: 1. UndiJiiigkvitcn am Autoklaven (Zu- und Ableirungen) wahrend der Polymerisation und beim Riickgewinniiugsveifahrcn; 2. bcim Ablassen und Offnen der Autoklaven sowie taint Tr.ik:mng-pr*.'zcl Jc krtigcti P\*Iyviu;dchlorida. Das Rcstgas besteht zu 9U~ aus Vinyldilorid (6) und fuhrt bei den Atbeitcm haufig zu bleiemer Miidigkeit und kurzzeitiger Desorientienheit; 3. aus den in den Polymerisatniedcrsdilagen der Reaktorinnenvand enthaltenen ICaverncn (5). Vom tedinischen Ablauf her bestunde also durdiaus' die Moglidikcit, dafi Vinyldilorid das hier erdrterte Ktankheitsbild auslosL Obex dieWirkungen des VinylchloriJs auf deamensdilidien und tierischen Organismus ist bercits einiges gesichert. Seine narkotische Wirkung ist seit langem bekannt (22). Ober angioneutotische Storungen bis hin zum Raynaud-Syndrom'bei Arbeitern der Vinyichloridhcrstellenden Industrie wurde seit 1.9JS wiedrrhult beriditet (1, 10, 21). Vcreinzelt finden sidi in der Literjrur'' Hinwcisc auf Intoxikationen mit Vinyldilorid, wobei reversibles Sdiwindclgefuhl, leichte bis ausgepragte Benommenheit, Desorientienheit, Hautersdieintuigen wie bei einer Verbrennung zweiten Grades und verminderte Blutgerinnung als Symptome frstgestellt wurden (g, 9, 11). Ein Azbeiter state an den Vcrgiftungsfolgen. Tierexperimentell konnten als Folge einer akuten Intoxikation mit Vinyldilorid Lungcnodem, Blutstauung in Lunge, Leber und Niere sowie eine verminderte Blut gerinnung nadigewiesen werden (20). In Langzeitversuchen lieSen sidi an der Leber zcntrolobulare Degene ration und Nekrosen mit schaumiger Vakuolisicrung sowie periportale zclligeInfiltration erzeugen(2g).Jungst wurde fiber Haut-, Lungen- und Knodienmmoren und Storungen im Knodienaufbau beriditet (27). Diese tierexperimentcllcn Befn-ide (iihrten dann 1970 errcut zu einer Senkung des MAK-(nuximale Arbeitsplaczkonzentration-)Wertes auf nun 100 ppm (13). Cber regelmaGige Gaskonzentrationsmessungcn am Arbeitsplatz ist uns bisher nidits bekanntgeworden. Die im Rahmen der gesdiilderten Krankheit angeffihrten Symptome: Raynaud-Syndrom, toxisdte Leberverandetungen, Storungen im ICnochenauibau und pranarkotisches Syndrom, sind also durch Vinyldilorid ausiosbar, so daG wir in Kennmis der Arbeitsplatzvrthaltnisse den dringenden Verdacht iuGern, daG langfristige Vtnylchiorid-Exposition zumindest eine, wenn nicht sogar die krankheitsausloscnde Ursadie ist. Weil auGerdem das Kranheitsbild durch Mitbeieiligung zahlreicber Organe den Charakter einer Systcmkrankheit erhalt und die Bezeidmung berufcbedingre Akroosteolyse* nur auf cin fur die Prognose richer nicht entscheidendes Symptom mnweist, liaben wir die ficzeidinung sogenanme Vtnylchlorid-Krankheit* vorgesdilagen. Pathogenese Die Pathogenese ist nadi wie vor ungeklart. Aufbauend auf unseren Bcfund^n sprechen wir aber die Vermutung aus, daG der Ktankhcit ais pathogenetisches Prinzip im weitesten Sinne eine Sturung am Bindegewebe oder GefaGbindegewcbe zugninJe liegt. A!s Hinweis auf eine zugrunde lie* gendc Storting am Bindcgewebe sahen wir die qualitatiren und quantitativen Vcrandcrungen des kollagenen nnd elastischen Bindegewebes der Haut, die Vetmehrung kollagener Fasem in den portalcn Fcldern der Leber, die kapillarmikroskopisch und histologisch in der oberen Cutis sichtbare Vermehrung und Erweiterung der Kapillaren ebenso an wie die klinisdt, angiographisdi und thcrmogrzphisch nadiweisbaren peripheren Durchblutungsstbrungen. Audi die Vcrandcrungen am Knocben .(Osteolysen als Folge trophischer Stoning durdi Sdiadigurg der Vasa nutricia) und der Lunge (restr'krive und obstruktive Ventilationsstorungen) konnten gieicbsinnig gedeutcc werden. Die Prognose dec Ktankheit -- in der Literatur bisher recht optimistisch beuneilt sdieiut uns im Hinblick au/ Thrombopenie, Splenomegalie und portale Hypertension ernst, und auch hinsiditlidi der Knoehenveranderungen solltc sic nach unseren Beobachtungen mit Zuriickhahung gestellt werden. Konsequcnzcn fur die Praxis r" ias Systemkrankheit, emsre Prognose und rclativ .dorbiditat (nach unseren Erhebungen bisher richer mehr als 10%) uberwiegend junger Manner veranlaGt uns, folgende Porderungen aufzustellen: 1. Durchfiihrung epidemiologisdier und praventivmedizinisdicr Untcrsuchungsreihen.
