I have described each problem below making references to various attachments. The attachments are numbered to correspond to the numbers mentioned in Che descriptions. (1) Work history dates for many departments on the personnel records are outside the range of the time frames given on the Product Department History sheets {Attachment 1). In most cases, the daces which appear on the personnel records are greater than the upper limit of Che time frame shown on Attachment 1. Rather than providing you with a listing of each occurrence of this problem, I have simply noted the departments on Attachment 1 (Dept. No. circled in red) which are associated with a time period inconsistency. Perhaps you could re check the time frames on Attachment 1 with someone at the plant. My feeling is that the work histories are correct and the time frames on Attachment 1 are not precise. (2) The following 29 department numbers appear on the personnel records but do not appear on the Product Department History (Attachment 1). Dept. Nos.: 40, 96, 104, 107, 109, 112, 114, 115, 117, 130, 133, 135, 137, 141, 142, 149, 173, 174, 210, 501, 509, 512, 513, 611, 713, 722, 809, 821, 904. I need to know whether or not these are valid depart ment numbers, if they are valid, what was the time frame of their existence, and if any of these departments were associated with formaldehyde exposures. The ex tent of the missing information is indicated beside each name. I have also enclosed copies of all the work history information that we do have on these men. I realize that you may not have any more information on these men, however, prior to analysis I would like to have this confirmed with you. If you do uncover any of the missing work histories please send me a xerox copy.
