DEMOLITION: Reporting to the EPA Region Administrator is required in all cases. A. If more than 260 linear feet or 160 square feet is involved, the nofification must include the following items and be delivered 10 days prior to commencing demolition. 1. Name of owner or operator of demolition operation. 2. Address of owner t>r operator. 3. Description of building or facility to be demolished. 4. Address of building or facility. 5. Starting and completion dates of demolition. 6. Nature of planned demolition methods. 7. Procedures to be employed to meet 40CFR 61.22 d and j. 8. Name and address of waste disposal site to be used. 9. Name, title, and authority of the state or local govern mental representative who has ordered demolition subject to 40CFR 61.22 (d)(6).
