Section I - INTRODUCTION
Albuquerque Bernalillo County Water Utility Authority (NM3510701) Inspection Date: 10/20/25 to 10/23/25
PURPOSE OF THE INSPECTION
EPA Region 6 inspectors Mariela Reyna, David Parker, Conor Goulding, and Jacob Lunn (hereinafter referred to as the EPA Inspection Team) arrived at the Albuquerque Bernalillo County Water Utility Authority (ABCWUA) at approximately 8:30 a.m. on October 20, 2025, for an announced Compliance Evaluation Inspection. We met with H. Warren, Cody Stinson, Danielle Shuryn, Jeff Pompeo, Michael Richardson, and Joel Berman. For a copy of the Opening Conference Sign-in Sheet refer to Appendix 1. I presented my credentials to the attendees at the opening conference and informed them that this was an EPA inspection aimed at determining compliance with the Safe Drinking Water Act (SDWA) Section 1433, which requires community water systems serving more than 3,300 people to complete a Risk and Resilience Assessment (RRA) and develop an Emergency Response Plan (ERP). The EPA Inspection Team also informed the system representatives that we would be conducting a cybersecurity evaluation alongside the Section 1433 review. To facilitate the inspection process, the EPA Inspection Team used a checklist outlining the requirements of SDWA Section 1433 during the document review.
FACILITY DESCRIPTION
The water system is owned and operated by the ABCWUA and serves a population of approximately 650,000 people. It is a community water system as defined by 40 CFR Part 141 Subpart A. The system serves the greater Albuquerque metropolitan area with a dual groundwater/surface water system. It consists of the San Juan Chama Surface Water Treatment Plant, four arsenic treatment plants for wells with elevated arsenic, 63 active groundwater wells, 31 disinfection facilities, 40 pump stations, 65 reservoirs, and 21 pressure zones. The system has a capacity of approximately 84 million gallons per day (MGD) of treated surface water and 170 MGD of treated ground water. Two additional arsenic treatment plants are being planned for some of the inactive groundwater wells. The surface water plant is offline during the summer months (June/July to November) when the Rio Grande River experiences lower water levels. To compensate for this, the system has developed an Aquifer Storage and Recovery System (ASR) which allows treated water from the treatment plant to be pumped underground for storage. This water can then be pumped back out during periods when the river is low, and the surface water treatment plant is not operating. This has also allowed for recovery of the underground aquifer, and in recent years the water table has increased. ABCWUA currently employs approximately 635 people and the water system operate 24/7.
Section II - OBSERVATIONS
The EPA Inspection Team inspected portions of the public water system (PWS), starting with the surface water intake on the Rio Grande River, and followed by the College Arsenic Treatment Plant and Reservoir on October 20, 2025. On October 21, 2025, the team inspected the San Juan Chama Treatment Plant, the laboratory, the Supervisory Control and Data Acquisition (SCADA) room, Corrales Reservoir #3, Corrales Well #8, and Corrales Well #2. On October 22, 2025, the team inspected the ARS Well, Griegos Well #1, Vol-Andia Well #1 Pump Station and Reservoir, Leyendecker Well #1, Glenwood

Reservoir, and Lomas Reservoir. On October 23, 2025, the team inspected Burton Pump Station and Reservoir and Burton Well #3.
Document Review
The EPA inspection team reviewed the System's Section 1433 RRA and ERP and the System's cybersecurity practices.
SDWA Section 1433(a), 42 U.S.C. 300i-2, Community Water System Risk and Resilience requires a CWS serving more than 3,300 persons to conduct an RRA of its system, including an assessment of:
a. the risk to the system from malevolent acts and natural hazards; b. the resilience of the pipes and constructed conveyances, physical barriers, source water, water
collection and intake, pretreatment, treatment, storage and distribution facilities, electronic, computer, or other automated systems (including the security of such systems) which are utilized by the system; c. the monitoring practices of the system; d. the financial infrastructure of the system; e. the use, storage, or handling of various chemicals by the system; and f. the operation and maintenance of the system.
Observation 1. On October 20, 2025, the EPA inspection team completed an inspection of Respondent's System under SDWA Section 1445, 42 U.S.C. 300j-4. The EPA inspection team requested to review Respondent's RRA and observed the following:
a. Document was available at the time of the inspection. b. The RRA was dated to show that it was completed before the date of the
certification of completion. c. The RRA did assess all required elements of Section 1433(a). d. The RRA did include a complete assessment of the required elements.
SDWA Section 1433(b), 42 U.S.C. 300i-2, Community Water System Risk and Resilience requires a CWS serving more than 3,300 persons to conduct an ERP of its system, including an assessment of:
a. strategies and resources to improve the resilience of the system, including the physical security and cybersecurity of the system;
b. plans and procedures that can be implemented, and identification of equipment that can be utilized, in the event of a malevolent act or natural hazard that threatens the ability of the CWS to deliver safe drinking water;
c. ZZZZZ malevolent act or natural hazard on the public health and the safety and supply of drinking water provided to communities and individuals, including the development of alternative source water options, relocation of water intakes, and construction of flood protection barriers; and
d. strategies that can be used to aid in the detection of malevolent acts or natural hazards that threaten the security or resilience of the system.

Observation 2. During the inspection, the EPA inspection team also reviewed Respondent's ERP and observed the following:
a. Document was available at the time of the inspection. b. The ERP was dated to show that it was completed before the date of the
certification of completion. c. The ERP did complete all required elements of Section 1433(b).
Observation 3. The EPA Inspection Team observed that the foundation seal for College Reservoir, Corrales Reservoir #3, Vol-Andia Reservoir #2, and Glenwood Reservoir was missing in areas. (refer to Appendix 2, Photographs 1, 3, 9 and 15). In the correspondence received on November 24, 202, ABCWUA states that new felt material will be installed during the low water demand period between January and April 2026 (refer to Appendix 4).
Observation 4. The EPA Inspection Team observed several dents on the College Reservoir outer walls (refer to Appendix 2, Photograph 2). The most recent storage tank inspection, completed on March 19, 2025, and submitted to the inspection team at the time of inspection, states that the structural elements and exterior coating of the tank are in fair condition.
Observation 5. The EPA Inspection Team observed a hole under the Vol-Andria Reservoir #2 (refer to Appendix 2, Photograph 10). The hole was immediately covered up, and photo documentation was submitted to EPA on October 23, 2025, as proof of completion (refer to Appendices 3 and 4).
Observation 6. The EPA Inspection Team observed plant roots at the interface of the tank bottom plate and tank foundation at Griegos Reservoir and Glenwood Reservoir (refer to Appendix 2, Photographs 6 and 15). The vegetation was immediately removed, and photo documentation was submitted to EPA on October 23, 2025, as proof of completion (refer to Appendices 3 and 4).
Observation 7. The EPA Inspection Team observed cracks on the concrete bases of Leyendecker Reservoir (refer to Appendix 2, Photograph 13) and along Lomas Reservoir #2 walls, under one of the pipes and near the sealant lines on top of the tank (refer to Appendix 2, Photographs 21, 22, 23, and 24). In the correspondence received on November 24, 202, ABCWUA states that the concrete ringwall integrity of Leyendecker Reservoir will be evaluated by a structural engineer in the spring of 2026, and the Lomas Reservoir #2 structural inspections and subsequent repairs are programmed to take place within the next five calendar years (2026-2030) (refer to Appendix 4).
Observation 8. The EPA Inspection Team observed concrete patches missing from the outer wall of Lomas Reservoir #1 (refer to Appendix 2, Photographs 18 and 19). In the correspondence received on November 24, 202, ABCWUA states the Lomas Reservoir #1 structural inspections and subsequent repairs are programmed to take place within the next five calendar years (2026-2030) (refer to Appendix 4).

Observation 9. The EPA Inspection Team observed minor rust corrosion of the Griegos Reservoir intake pipe, the Vol-Andia Reservoir #1 and #2 intake pipes and the Leyendecker outflow pipe, as well as on the bottom of the tank and in circles going up the wall of the Reservoir (refer to Appendix 2, Photographs 7, 8, 11, 12, and 14). In the correspondence received on November 24, 202, ABCWUA states that the exposed pipe will be recoated during the low water demand period between January and April 2026 (refer to Appendix 4).
Observation 10. The EPA Inspection Team observed minor rust on one of the pipes connected to a booster pump at the Glenwood Pump Station (refer to Appendix 2, Photograph 16). In the correspondence received on November 24, 2024, ABCWUA states that the surface rust will be repaired between January and April 2026 (refer to Appendix 4).
Observation 11. The EPA Inspection Team observed significant rust corrosion on the bottom of the salt storage tank at Corrales Well #8 (refer to Appendix 2, Photograph 4). In the correspondence received on November 24, 2024, ABCWUA states that the salt shed will be replaced at the end of its service life, which it has not yet reached (refer to Appendix 4).
Observation 12. The EPA Inspection Team observed sealant peeling from the top of the Lomas Reservoir #1 (refer to Appendix 2, Photograph 17). In the correspondence received on November 24, 202, ABCWUA states that the Lomas Reservoir #1 structural inspections and subsequent repairs are programmed to take place within the next five calendar years (2026-2030) (refer to Appendix 4).
Observation 12. The EPA Inspection Team observed vegetation growing on the top of Lomas Reservoir #2 and stalactites hanging from under its lip (refer to Appendix 2, Photographs 20 and 25). In the correspondence received on November 24, 202, ABCWUA states and shows through a photograph that the vegetation was removed on November 24, 2025 (refer to Appendix 4).
Observation 13. The EPA Inspection Team observed a leak in the discharge valve from the sodium hypochlorite tank (refer to Appendix 2, Photograph 5). It was immediately addressed by the facility.
Observation 14. The EPA Inspection Team observed insulation peeling from the roof of the sodium hypochlorite building at the Burton Pump Station (refer to Appendix 2, Photograph 26). In the correspondence received on November 24, 202, ABCWUA states, and shows through a photograph, that the insulation was reinstalled on November 21, 2025 (refer to Appendix 4).
Observation 15. The EPA Inspection Team observed vegetation growing inside the Burton Well #3 wellhouse (refer to Appendix 2, Photograph 27). In the correspondence received on November 24, 202, ABCWUA states, and shows through a photograph, that the vegetation was removed on November 21, 2025 (refer to Appendix 4).

Section III - CLOSING CONFERENCE EPA Region 6 inspectors Mariela Reyna, David Parker, Conor Goulding, and Jacob Lunn conducted a closing conference at ABCWUA at approximately 11:30 a.m. on October 23, 2025, for the inspection. For a list of those present at this meeting, refer to Appendix 1. During the closing conference, the EPA Inspection Team reviewed the preliminary observations identified during the inspection, fielded questions from facility personnel, and provided information about the next steps in the inspection process.
