Code: 101-5156 PREVENTIVE CARE AND TECHNICAL PRECAUTIONS IN PLANTS PRODUCING PVC R&S 131492 The article deals with the prophylaxis and teohnioal pre cautions concerning work with VC (vinyl chloride) in PVC (poly vinyl chloride) producing plants. Those technical guidelines created by expert physicians in cooperation with the -Employers Liability Insurance Association (BG) of the chemical industries and with the aid of safety engineers and chemists are particu larly stressed. Reference is made to those groups in the Federal Republic of Germany that concern themselves with the so- called VC disease and also to urgently needed international cooperation. PVC is not a luxury good and cannot be replaced by another product. However, the further polymerisation of VC and PVC can only be defended when technical precautions or per sonal precautions eliminate any health dangers. \ 1. Introduction, historical development and resulting questions Health dangers in plants producing PVC, probably caused by vinyl chloride, have been known to industrial physicians abroad since the beginning of the sixties [2-6,8,12,19,20,22,23]. But at the "Medichem" 1972 symposium of industrial physicians of the chemical industry in Luwigshafen, Juhe and his assistants [1,11,19] and Lefevre [13,14,18,21] still reported exclusively about acroosteolysis and sclerodermic diseases which had become known during the previous years in Germany and abroad with workers in PVC plants. The work group around Veltmann [11,12], reported in the following months on increasingly extensive and serious ail ments in Germany caused in all probability by the effects of vinyl chloride. The alarm was sounded in the Federal Republic of Germany at the beginning of 1974 with the report of 24 cases of angiosarcoma around the world. Three of these cases were found in PVC plants in the Federal Republic of Germany. The results w re a worldwide exchange of experiences and extensive animal Pag 2 experiments conducted by experts in areas where no harmful PVC effects had been noted. The experiments in all instances were concerned with detecting the cause of the disease. At the same time, existing and needed preventive care and technical pre cautions in PVC plants were reconsidered. 2. Preventive care and technical precautions in PVC plants Those workers who had contact with certain chlorinated hydro carbons in their work had always been subjected to regular check ups. Since the report of acro-osteolysis cases around the world, an X-ray of the hands and feet and more recently other areas of the bone structure were included in the check-ups. Following the disease symptoms of the so-calldd VC disease, increasingly mor complex laboratory and clinical tests were needed. R&S 131493 In the following sections we shall show 2.1 2.2 2.3 The industrial preventive care and technical precautions in PVC plants, ^ The preventive care and technical precautions taken outside the plant in the Federal Republic of Germany, and the International industrial preventive care and technical pre cautions that were taken after the reports of the serious"~socalled VC disease. 2.1 Industrial measures 2.1.1 Creation of an interdepartmental VC-PVC work group, con sisting of a representative from the VC-PVC producers' division, th Works Council, the safety department, the Toxicology Depart ment . and the Industrial Medical Department. The goal of this work group was to draw up proposals to improve occupational hygiene and technical measures to decrease exposure to VC. They also were to reconsider the interdepartmental work guidelines. Pag 3 2.1.2 Instruction of the total work force in the VC-PVC plants on the toxicity of VC was accomplished with the aid of the Works Council. These conversations had to be conducted in a very inten sive manner due to the great publicity this subject had received. The necessity for more complex check-ups and epidemiological tests had to be pointed out. We recommend the use of an interpret r if foreign workers are among the employed. 2.1.3 Extension of the industrial medical preventive examinations to the recruitment, aptitude, and check-up examinations" We are going to deal with this more extensively under section 2.2.2.1, Employers Liability Insurance Association guidelines for industrial medical examinations. 2.1.4 Extension of technical precautions Here we have taken into account the guidelines of the BG of the chemical industry. One of the preconditions for introducing technical measured in plants to reduce the exposure to VC is a suitable method to measure VC in the air of the area concerned (1-100 ppm) (Figure 1). Among the available suitable measuring methods three stand out: a) The infrared absorption process (GRAS). This process does not work reliably in the especially interesting range below 50 ppm. b) The Ionoflux process has good sensitivity particularly for chlorinated hydrocarbons; but it also has the disad vantage of a long reaction time (approximately 5 min.). c) The Flame Ionization process (FID) has good sensitivity, a quick reaction time, but a low bias for chlorinat d hydrocarbons. Which method b) or c) is chosen by the different PVC plants depends upon their special circumstances. Both methods are being used in the Federal Republic of Germany. R&S 131494 Page 4 The following technical measures have been introduced to de crease exposure to VC: a) Increased ventilation, e.g. improved exhaust of the auto claves to be cleaned (Figure 1). b) Elimination of the so-called preferred points of leakage through changes in the sealings and installation of ef fective ventilating systems. c) In special cases improvement of the independent air cooling through increased ventilation in clearance spaces. d) Utilization of breathing protection devices and protec tive clothing during extended cleaning operations. It is the declared goal of the PVC producers in the Federal Republic of Germany to achieve automatic autoclave cleaning in the shortest possible time (Figure 3). Until then a strict time limit is recommended for the cleaning process. 2.2 Preventive care and technical precautions outside the plant in the Federal Republic of Germany 2.2.1 Foundation of three works groups in the VKE-association of plastics manufacturers: 2.2.1.1 2.2.1.2 2.2.1.3 for the- production of VC and PVC for the industrial medical service and toxicology for technical precautions The main concern of these work groups is to obtain comprehen sive information from those projects dealing with the area of VC toxicity. At the same time they engage in an exchange of ideas about common guidelines for the production and manufacturing of PVC with the goal to decrease the VC content as much as possible. These goals do not eliminate the necessity to clarify the causes of the problems. R&S 131495 R&S 131496 Page 5 2.2.2 Guidelines of the Employers Liability Insurance Association The general association of th industrial Employers Liability Insurance Association is the publisher of these guidelines pro duced by the Employers Liability Insurance Association of the chemical industries. The guidelines are divided into those con cerning the plant physician and those concerning production. 2.2.2.1 Guidelines for effective industrial medical preventive care examinations These guidelines have been created by experienced state in dustrial physicians and industrial physicians with the assistance of the Employers Liability Insurance Association and contain regulations concerning the medidal examination at recruitment, i.e. the examination before beginning work in an area endangered by vinyl chlorides, and guidelines for medical examinations during work. These guidelines mention the danger posed by vinyl chlorides but do not include the fact that it has not yet been d termined that the vinyl chloride monomer is the sole cause for the so-called vinyl chloride disease. The first check-up is to be conducted after six months and further medical examinations are determined by the amount of exposure, i.e. they are to be conducted after six or twelve more months. Where there is any doubt, additional extensive clinical examinations are called for after consultation with the state industrial physician and the proper insurance carrier. According to the examination results the following medical evaluations are to be passed on to manage ment: suitable, conditionally suitable, permanently not suitable, not suitable for a limited time period, or not suitable. These guidelines also contain references to symptoms of the so-called VC disease, to its physical and chemical characteristics, to the MAK-values of VC, and to the occurrence and dangers of the sub stance. The occupational prohibitions concerning juveniles in accordance with Section 15, Article 2 and Section 4 of the Mother Protection Law together with Section 15, Article 3 concerning the 16vvev Pag 6 r gulations about dangerous occupational substances should also be noted. The time intervals for medical examinations found in th guidelines have also been adopted in the general regulations (Section 18) of the Employers Liability Insurance Association of the chemical industry. 2.2.2.2 Guidelines for technical precautions At the beginning of 1974 the BG of the chemical industry issued regulations concerning the precautionary measures to avoid health hazards in working with vinyl chlorides. These regulations lay down general requirements, protective measures^ concentration measurentents and particularly technical requirements for the cleaning processes. The general regulations deal with work within the currently used MAK-value as well as with the instruction of the workers about the dangers in working with vinyl chlorides, the safety regulations and the working regulations. The protective measures include the ventilation and exhaust systems as well as the utiliza tion of breathing protection devices during repairs. The concen tration measurements are also regulated. The results are to be r corded and kept on file for at least five years. The technical and protective measures during cleaning opera tions are described in a very detailed manner. 2.2.3 VC/PVC information exchanges in the Federal Republic of Germany The order in which the following points appear does not imply any judgment on their relative importance: 2.2.3.1 Work group of the state industrial physicians of Nordrhein This work group is directed by ltd. Reg.-Gewerbemedizinaldirektor Dr. Reini and includes the state industrial physician, industrial physicians and the BG of the chemical industry and chemists. Page 7 2.2.3.2 ASK-Work group This is the expert commission for industrial medicine of the BG of the chemical industry. 2.2.3.3 The expert work group of the BAM - drawing up the VIII.BKVO This work group also deals with the problems of VC-PVC. 2.2.3.4 Allocation of research contracts The BAM, the regional governments and the BG of the chemical industry have given research contracts for the investigation of the so-called VC disease to Prof. Henschler, Prof. Lehnert, ltd. Their work is concerned with the establishment of basic principles about the investigation of the so-called VC disease. 2.3.7 Exchange of ideas between the industrial physicians in separate PVC plants A continuous, informal exchange of ideas about the so-called VC disease takes place independent of the work groups mentioned above. 3. About the MAK-value of vinyl chlorides In all work areas in which VC is used, health hazards arising from technical or personal precautions are to be eliminated. When this requirement will be fully met cannot yet be determined. The MAK-value of VC, set at 500 ppm just a few years ago in Germany, was lowered to 100 ppm in 1973 and later to 50 ppm. The MAK-value list of 1974 does not include a MAK-value for vinyl chloride any more because vinyl chloride has been added to the group of substances causing cancer in animals and probably also in humans. A work group of the Committee for Dangerous Occupational Substances is going to determine a technical concentration limit for VC. Until that is done, a VC concentration of 50 ppm in all work areas should not be exceeded. All PVC producers in the Federal Republic of Germany are working for a further reduction of the recommended concentration level. However, it cannot yet be determined if technical requirements are going to permit a reduction to 25 ppm, 10 ppm or even less. R&S 131499 Page 9 4. Summary PVC in the plastics industry cannot be replaced by another product. PVC is needed in today's economy and is not a luxury article. Animal experiments have been conducted worldwide since May of this year using exposures of 5 ppm VC and lower. The further production of PVC through the polymerization of VC can only be justified if technical safety devices and personal pro tective measures exclude any health hazard for the employees. The utilization of VC in the form of sprays or as a carrying agent for drugs has been discontinued. Hie so-called VC disease has been accorded a great amount of attention in technical and particularly the popular literature. PVC producers, chemists, safety engineers, epidemiologists and industrial physicians from around the world are engaging in an intensive exchange of ideas. This international cooperation has proven useful and legislators O-f the western world will be able to base their decisions concerning the further use of VC in PVC production on internationally obtained research findings. Presently the question is being examined as to what extent health hazards appear in the manufacturing of PVC and what protective measures are necessary. * l References J AJam*. II*. Ein weltwciter Erl'ahriingsaustausch sowie umfangreidic Ticrvcrsuchc audi von Sadikennern durchgcluhrt, in deren Arheitsgcbict kciiic sog. VC-Schadigungen bekanntgeworden sind. war die Folgc. Die Fragcstcllung gait ubcrall dcr Erntittlong dcr Ursadio der Erkrankungen und glcichzcitig audi deni Uberdenken der vorhandenen odcr neu zu sdiatTendcn werksSrztlidicn Vorsorge und dcr ledinisdien SdiutzmaGnahmen in PVC-herstcilenden Uctricbcn. 2.13 Erweilerung der werkxdrzilidien Vorsorge-Uniersudinngen hei der Einsiellungs-, Eignungs- bzw. VC-Erkrankungcn ergrilTen wurden. 2.1 Belriehlidie Miipnaiunen 2JI Criindung eines innerbetrieblidicn VClPVC-Arheitskreists, bestehend aus eincm Vertreter der VC/PVC-Produktion, cincm Vertreter dcs Bctricbsrates. dcs Sichcrheitswesens, dcr toxikologisdicn Abtcilung und dcr werksSrztlichcn Abtcilung. Die Ziclsetzung dieses Arbcitskrcises besteht darin, allc nur denkbaren Sdirittc zur Verbcsserung der Arbcitshygienc, incl. tcchnisehc MaUnahmcn zur Expositionsvcrmindcrung gegeniiber VC sowie ObcrprOfung dcr inncrbctricblidicn Arbeitsriditlinicn in die Wcgc zu Icitcn. 2.12 Die Unierridilung der Gesiintthelegsduift in den VC/PVC-Beirieben hinsiehtlieh dcs Standes zur Toxizitiit von VC crfolgte untcr Hinzuzichung dcs Uctricbsralcs. Die Gespriichc miissen im Hinbliek aut die grolie Publizitiit des Thcnias intensiv gctiihrt warden, wobci audi aut' die Notwcndigkeit dcr unilasscndcr gewordenen (Jberwadiungsuntcrsudiungen sowie die Durdil'uhrung cpidcmiologisdicr Untcrsudiungen hingcwicscn wird. In 'dicsen Grundsiit/en wird von (icfahrdung durch Vinyldilorid gcsprodien -- unabhangig von der Tatsadic. daB bishcr nidit cindeutig gekliirt ist. ob das mononicre Vinyldilorid lalsiidilidi die alleiuigc* Ursadie. fUr die sogenannte Vinyldiloriderkrankung ist. Die idlgemeinen Anfordrrungen bcinhalten das Arbeiten innerhalb dcs dcr/citigen MAK-Wertcs fsouie die Untcrriditung der Beschaliigien iiber die Gefahren im Uni gang mit Vinyldilorid. iiber die Sidicrheitsbcstinimungcn und Betncbsanweisungen. Die .SebutzniaBuahmen bcin halten die Bclultung b/.w. Absaugung sowie die Verwendung von Atemsduitzgeriiten bei Rcparaturen. Die Konzentralionsniessungen werden vorgesdirieben. VCKrankheit ausgearbeitet. 23.7. Erfahnaigsuiisinnxiii zwixrhen den einzelnen Werkx* Srzten in PVC-herstellenden lietriehen Unabhiingig von den oben benannten Arheitskreisen lin det auf frcundschaftlicher Ebenc ein laulender Erfah rungsaustausch zu der sog. VC-Erkrankung stall. 3. Zum MAK-Wert von Vinyldilorid In alien Arhcitsbereichon. in denen VC verarheilet wird. ist cine gesundheilsschadigende Einwirkung der Suhstunz durch tcchnische Sidierhcits- Oder personlithe SchutzmaQnahmen auszusdilielkn. Wann diese Forderung mit hundertprozentiger Sidierhcit crlullt ist, kann z. Z. noth nicht gesagt werden. Der MAK-Wert von VC, der in Deutschland noth vor wenigen Jahren hei 5IN) ppm lag. wurde 1973 auf KXI ppm und spiitcr auf 50 ppm hcrahgesetzt. In dcr MAK-Wcrt-Liste fiir 1974 wird fiir Vinyldilorid ein MAK-Wert nicht niehr angcgchen werden. da Vinyichlorid in die Gruppe dcr fiir das Tier kanzerogenen Sub* stanzen eingcrciht worde i_ und mil groUcr Wahrsdieinlichkeit auch fiir den Mensdicn kan/crogen ist. Wie beim Benzol wird durch cinen Arheitskreis des Ausschusscs fiir gefiihrlidie ArheitsstolTe cine tcdinisdic Kidilkon/entration fiir VC festgesetzt werden. Bis dahin ist anzustrehen, daB die VC-Kon/entration von 50 ppm in 4. Schlultfolgeruiigeii PVC 1st durdi ein anderes Produkt dcr Kunststolfindustrie nidit zu ersetzen. PVC wird von dcr Wiri.sdiall heute gebruudil und ist kein l.uxusgcgenstund. Seit Mai dieses Jahres werden in der Welt unil'angreidie '1 ierversudie bci eincr F.xposition von bis zu 5 ppm VC und darunter durchgeluhrt. Die weitere Herstcllung von PVC durdi Polymerisation von VC ist aher nur dann zu verantworten, wenn die tedinischcn Sidierhcitseinriditungen und pcrsonlidien SdiulzmaUiiahmen' cine gcsundhciilidic GcfiihrdUng der Miturheiter aussdilicBen. Die Verwendung von VC in Sprayfoim oder als Antricbsmiitel liir Arzncien ist eingestelll worden. Die sog. VC-Erkrankung hat in dcr Fadi- vor allcm aber auch in dcr l.aienpresse ein groOcs Echo gclunden. PVCHerstcller, Cheiniker. Sidierheitsingenicure, Epideniiologen und Werksarzie sind in der ganzen Welt in einen engen Erfahrungsaustausdi getreten. Diese intcmalionale Zusammenarbeit hat sidi bewiihrt. und die Gcsetzgeber dcr westlidtcn Welt werden ihre Entsdieidungen hinsiditlich dcr Weitervcrarbeitung von VC zu PVC von den in ternational erhohenen Uniersuehungsergebnissen abhanyig machcn. Zur Zeil wird audi die ITage gepriift. inwicwcit gcsundhciilidic Sdiiidigungen hei dcr Weitervcrarbeitung von PVC aultreten kdunen und entsprediende Schut/niaOnahmen notwendig sind.
