Please note the following: (1) Documentation was provided on only those shipments for which billings payments varied from the Burlington/ Flemington rate in effect at the time of shipment. (2) Documentation for over-payments made by Cass Bank consists of only copies of freight bills and bills of lading. (3) Firestone was billed collect on three shipments out of Norco and reimbursed by Teimeco at $1.53/cwt.
