Harris SUBJECT: SAFETY RULES REFERENCE- A copy of the Safety Rules for Outside Vendors at the Azusa Plant is attached. The rules are not all encompassing but are a reminder to outside people that they are in a work area that can be hazardous, and-they should get help and have the area checked by knowledgeable supervision before they find themselves in an unsafe condition. Any Purchase Order for labor within the plant should mention that compliance with the safety rules is a part of the P.O., and two copies of the rules should accompany the P.0. The shift supervisors and purchasing should maintain a supply of copies of the rules to pass out to any vendor or workman that-wants them. Since neglect of these rules may endanger lives and property, compliance should be enforced by reminding the offender of the rules and the hazard created; if that is not adequate to obtain immediate compliance, the individual or crew should be removed from the work site, and management of the vendor should be advised of the situation. Delgado 0140 SAFETY RULES FOR OUTSIDE VENDORS AT THE AZUSA PLANT 3/30'75 Since infractions of these rules may endanger lives and property, offenders will be sent from the plant. 1. The crew foreman must report to the shift supervisor at the beginning of each shift. 2. No smoking is permitted in the plant, except in designated areas. 3. Safety glasses, hard hats, shirts, long pants, and shoes (no canvas shoes) must be worn at all times in all working areas. 4. A Satety Work Permit must be obtained from the shift super visor before disconnecting hoses or pipes where pressures may exceed 15 psig, before any w.elding, cutting or open fire operations, before working inside any vessel or enclosed space. 5. Chemical splash goggles must be worn when breaking into any product or utility pipeline or when working in any area marked with a "Wear Goggles" sign. 6. A fresh air respirator must be worn when working with PAA or MAA lines or tanks. 7. A cartridge type respirator is required for vapor and acid areas. 8. Lockouts must be installed at all times when working on any machine or system that is electrically powered. 9. Only authorized persons are allowed to drive Cyanamid's fork lifts. 10. Practice good housekeeping. Keep the work area clean and orderly. Clean up after each job. 11. Contact the shift supervisor for any questions. 0141
