Comments on Statistics - Mark Riddle There were no recordable injuries in February and none so far in March. SC 13811 STC 2430617 LAM015560 PMSB Meeting Minutes - March 21, 1984 Page 2 We have now gone 139 days since the last recordable injury on November 3, 1983 and we have also gone 554 days since the last Lost Work Day Case on September 14, 1982. We should reach the 5,000,000 safe manhour mark around June 15, 1984. Areas having a high or increasing cumulative monthly injury rate are: Carpenters Pipefitters Dept. 44/45/46 Dept. 48/56 Dept. 50 Dept. 53 Areas showing good or improving injury rates are: Power 2 Painters HCN Complex Electricians Areas having excellent safety records are: Insulators Dept. 513 - 7 months without an injury - 18 months without an injury The Instrument Dept, had an injury in February breaking their streak of 11 months without an injury. C. Contractor Safety - Jeff Peterson There were 6 total injuries - one of them was an RI-Lost Workday Case. This injury involved an Opel pipefitter who sprained his ankle while stepping down from a crane. Details are contained in Incident Report C-84-01. D. CEP Safety - Jeff Peterson There have been no injuries year to date. II. REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC) - G. T. Ryan A. In response to a recommendation from Incident No. TC 84-1, SAC is to develop a definition of ownership in operation or responsibi lities for ex-battery limit pipelines and systems with SHAC and make a recommendation on policy. B. There was a discussion about the new general permit P-0110 which has not been issued and the work permit for off-battery work and use of heavy equipment near electrical lines P-0118. It seems that SC 13812 STG 2430618 LAM015561 PMSB Meeting Minutes - March 21, 1984 Page 3 most people feel that P-0118 deals with just electrical work near overhead lines and not for off-battery maintenance work. SHAC is to look at these two permits with the intention of making P-0118 into work involving electrical equipment near electrical lines and combine the other off-battery work into the general permit procedure. C. There was a discussion about the ground rules for special permits. This is currently as follows: "The final approval level cannot be delegated below the Manufacturing Superintendent or equivalent (level 36). Authority can be delegated laterally or up in the absence of the Manufacturing Superintendent." Gene Tromblee said that he would review this policy with SAC in another meeting. III. REPORT OF THE PLANT SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC) E7T: Bailey------------------------------------------------------------------------------------------------------- A. Contractor Safety Program Rufus Baca handed out the Committee's proposal for the contractors' safety program. Comments on the program are to be returned to Baca by 3/28/84. He also handed out a Contractor Performance Appraisal form that would be utilized at the end of each contract to be critiqued also by 3/28/84. Gene Tromblee asked if the MFI guideline for contractor safety had been approved at this date. Baca is to check on this. The intent is to have the program comments in by 3/28/84 and to MFI legal for review by 4/1/84. B. Hourly Safety Committee Meeting - February Casey Yauger was elected chairman of the Hourly Safety Committee. A discussion of the policy of leaving keys in the vehicles parked out in the plant was held. SHAC's recommendation was to continue this policy of leaving keys In the vehicles when they are out in the plant except in those areas where there js designated parking spots and where the vehicle would not interfere In the event of an emer gency. C. Quarterly Safety and Housekeeping Inspection Departments who have not responded to the 4th Quarter Inspection are as follows: 13813 Dept. 44 Dept. 51 Power 2 Dept. 50 Stores Labs/Offices Warehouse STG 243061 LAM015562 PMSB Meeting Minutes - March 21, 1984 Page 4 There was a discussion as to whose responsibility it is to reply for the office areas and SHAC is to investigate this and assign a person responsible. Schedule for the 1st Quarter Inspection is during the weeks of April 2-13, 1984. The goal is to determine plant safety involvement on an individual level. Emphasis areas: Previous inspection followup, hearing conservation, relief valve testing records, emergency procedures, AVIP records, and overspeed protection records. Gene Tromblee asked that we move up the timing on these inspections to ensure that we get them in the right quarter and that the next quarter inspection should be totally unannounced. D. STOP Program Enhancement SHAC was asked to develop a program to enhance the STOP program. It is as follows: Use existing STOP cards Fill out information on back Process data by each General Superintendent Develop trends for each area Correlate trends to injury statistics Improve front end management of safety This was approved for trial basis. Each department is to handle their own statistics in conjunction with SHAC. E. Safety Shower and Eyebath Freeze Protection An interim report will be made at the April PMSB meeting. F. Plant News Safety Articles There have been several articles in recent plant news on safety in the various departments or crafts and this is an ongoing project. G. (Supak) STATUS OF S-T-O-P PROGRAM The first unit of STOP training was given to approximately 75% of the maintenance and operation personnel. The second unit of STOP training is scheduled to begin in April. VI. MISCELLANEOUS A. General Permit Procedure This is to be looked at for one final review in conjunction with P-0118 by SHAC. B. Bicycle Safety There seems to be two separate issues involved in the current bicycle controversy. One is the policy of riding bicycles to and from the gate. SAC is to work with Hagood on a decision regarding this policy of riding bicyles to and from the gate. The second is our current policy of requiring the wearing of hard hats while riding bikes in the plant. SAC is to review this policy. National Safety Council's "Award of Honor" Our plant received the National Safety Council's highest award the Award of Honor - for its performance for 1983. This is the first time we have won this award since tne new recordkeeping system went into effect in 1977. VII. CONTRACTORS' SAFETY PROGRAM - Rufus Baca See comments in Item III. A. VIII.
