Type and Purpose of Inspection Evaluation of Compliance with Construction General Permit IV. Facility Description The site is a construction site for a new office / garage building for Lunenburg Water District. It lies south of and uphill from Mass Ave, with a single driveway leading up to the building & parking lot. To the west of the building, up a short rise, is an unpaved staging area. The site slopes down to the south. The southern section is below the terrain to the east and west, while the northern section slopes down to Mass. Ave. and is more or less the same height as terrain on either side. A stormwater basin lies to the east and slightly south of the building. At the time of the inspection, the project was nearly complete. The building was complete and occupied, and most of the site had well - established grass or hydroseed. V. Inspection Mr. Spejewski called the facility in the afternoon of December 14 (the day before the inspection) and reached Christie Schwinger. Mr. Spejewski arranged to inspect the facility the next morning. As Mr. Spejewski did not know the exact time he'd be at the facility he arranged to call the facility when he arrived, and Ms. Schwinger would meet him outside with relevant paperwork. On December 15, Mr. Spejewski arrived at the site about 10:30, called Ms. Schwinger and met her outside the building. A. Opening Conference / Records Review Mr. Spejewski presented his credentials to Ms. Schwinger and re - iterated the purpose of the inspection. Ms. Schwinger explained the project, saying that the Water District had acted as their own general contractor for the project, doing most of the site work themselves. Mr. Spejewski asked to review the Stormwater Pollution Prevention Plan (SWPPP) and the records of inspections of site erosion controls. Ms. Schwinger said that, due to the recent move of offices, she had been unable to locate the SWPPP or reports. Mr. Spejewski requested that she email copies to Mr. Spejewski, and she agreed. [See addendum below.] B. Facility Tour 2 Commented [RT1]: Andrew, should the northern section be singular? ED_019088A_00020038-00002 Mr. Spejewski and Ms. Schwinger left on a tour of the site, and were joined by Fran McNamara, superintendent of the Water District. The building appeared complete, as did the parking lot on the east and north side and paved drive on the west. The building and parking were surrounded by well - established grass. To the west of the building, a short grass slope led up to an unpaved staging area, itself below the terrain to the south and west of the site. East of the building, a drainage swale led north to a stormwater basin. The swale was constructed of dirt and the drainage in the center was slightly eroded in places. Mr. Spejewski noted to Ms. Schwinger that some rip - rap or gravel could stabilize that and prevent sediment build - up in the basin. The basin had walls of dirt with well - established grass. A rip - rap wall divided a forebay from the other half of the basin. Ms. Schwinger said the basin discharged to the north, near Mass Ave. The slope down from the basin to Mass Ave. was grass and / or hydroseed. At Mass Ave., a pipe discharged east from the driveway to a swale / drainage along Mass Ave. Ms. Schwinger said it led through a culvert going north under Mass Ave. just to the east. On the west of the driveway, a short stretch of grass led to the property line, with a grassy area on the property adjacent to the site. A line of silt fence was in place along the site border, though now fallen down in multiple places. There was no sign of sediment carried into the adjacent cleared area (possibly a wetland near the road). Mr. Spejewski mentioned that the silt fence should be removed at some point, Ms. Schwinger said they were waiting for spring. At the top of the slope, west of the drive was a section that had been hydroseeded, but no grass was growing yet. Mr. Spejewski noted that attention needed to be paid to this area in the spring, as the binder in the hydroseed might not hold the area in spring. D. Closing Conference Finishing the tour, Mr. Spejewski thanked Ms. Schwinger and Mr. McNamara for their time and left the site at approximately 11:00 AM.
