SAFETY ADVISORY COMMITTEE -- Daues The committee has a number of items underway; a brief status report follows: A. Common Carrier Wrecks SC This item deals with the problems and procedures to be followed when Monsanto shipments are involved in traffic accidents. George Bostick is assembling information from a wide variety of sources for a policy review. Cole Downing has a similar project underway at the corporate level. B. In-Plant Railroad Safety Task Force Report This report is currently being reviewed by SAC. A number of the recommended changes have been made. Those involving significant capital approvals will be reviewed in depth prior to the next step. C. Nitrogen Blanketing of Internal Floating Roof Tanks The committee is waiting on a report by Jack Carlisle on the conference held on March 24th with representa tives of several other MCIC Plants. D. Silica Safety Association The Association asked for financial assistance to obtain definitive information to supply OSHA regard ing sand blasting exposure hazards and costs. The SAC recommended that a grant be given this association for this purpose. E. SHAC Action Request -- Use of Electrical Heating Tape. The SAC is aware of the SHAC actions but would like some additional information concerning the extent of the problem. Since the SHAC survey is still in progress this item will be placed on the agenda for the next meet ing. F. West Gate Traffic Problems related to traffic at the West Gate have abated somewhat since two entrances have been provided. Con struction in the area is temporarily delayed so the problem may be expected to occur again when construction is resumed. G. Warden Communications The SAC is monitoring this during fires and spills. The responsible persons are working on the problems and the committee regards this as an on-going development. More Plectrons and perhaps more two-way radios may be re quired to improve the communications with wardens during plant emergencies. H. Communications Emergency Procedure Conference Room The first proposal for out-of-plant emergencies communi cation center was rejected because of the high cost. A more modest proposal is being prepared by the Motorola Company. I. Construction Supervision Transportation. SAC rejected the proposal for a new type of transportation for in-plant transport of CED supervisors. Discussions are continuing between Ralph Martini and the committee to try to work out a compromise. SC 14134 STG 2430979 LAM015923 I ) Plant Manager's Safety Board Minutes - 4/20/77 3. 3. SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC) -- Stuart The following items were outlined by Bob Stuart. The details of these items are included in the SHAC Committee minutes. A. Guest Housekeeping Inspection will be held the week of May 19. B. The Office Safety Committee plans for their next program were reviewed. C. Safety signs at both gates will not be revised immediately. 4. REVIEW OF INCIDENTS The following incidents were reviewed by the Committee: A. Incident - Sulfuric Acid Spill - March 16 -- Stuart B. Near-miss Muriatic Acid Incident, Department 16 - March 20-- Stacy. C. Incident - Methanol Tank Car Spill - March 23 -- Stuart. The PMSB asks that this incident be reinvestigated and a new report be issued. D. Ammonia Spill - March 26 -- Danner. An investigation team has been formed and a detailed report is being prepared. E. AN Contamination of Condensate System - March 30 -- Ryan. An investigation team has been formed. Detailed study of the plant-wide impact is being defined and a report is being prepared. Several comments from the PMSB were made and the PT representative requested proper notice to PT when these events occur because they use condensate in their bench unit systems. The PT Safety Committee will highlight the need to prevent use of condensate in making coffee. F. Drott Crane Incident. On reviewing the incident report the PMSB was not satisfied manufacturing supervision was properly represented in the investigation and that the nature and extent of the hazards were not properly considered in the investigation. Apparent Ammonia Contamination of HDPE. Daues reported that there are some indications that Ammonia contamination of the nitrogen system occurred on several occasions recently in Polyethylene- The evidence is inconclusive, but he urged everyone to be alert to the possibility of contamination upstream of our fence line. As soon as his information has been completed Daues will issue a memo to PMSB. 5. PLANT-WIDE SAFETY PROGRAM Arnold Glass and Bob Stuart reviewed the next plant-wide safety program "Take the Extra Step for Safety". Schedule for kickoff is on May 16th. Copies of the program will be distributed to department heads for use in training their people on what to do during the program. 6. SAFETY AND PROPERTY PROTECTION SURVEY. -- TEXAS CITY PLANT. Members of the PMSB were urged to send their comments on the program and plans for overcoming the citations made in last year's S&PP survey to Arnold Glass as soon as possible.
