Turner May 24, 1985 Disciplinary Suspension At approximately 1:30 a.m. on May 23, 1985, VCM was released through the old module dump sweco while you were charging D-300 reactor and dumping D--400 reactor This VCM release occurred because the water charge valve was also open on D-400 reactor. When Don Johnson, Shift Supervisor, and I asked why you started charging D-300 reactor without first verifying the position of the D-400 water charge valve, you responded that there had not been a closed indication on the valve all night. However, you also acknowledged that you should not have charged the reactor without first verifying the position of the valve. The absence of a closed indication for this valve earlier on the shift did not relieve you of your responsibility to verify the position of the reactor valves. The Lead Operator is responsible for the safe, efficient, and environmentally sound operation of the unit. This VCM release was the direct result of your negligence. I am placing you on disciplinary suspension for two days; June 2 and 4, 1985. You should return to work on your normal schedule on June 5, 1985, at 7 p.m. I hope that you will take advantage of this opportunity to reflect on your job responsibilities and the importance of closely monitoring reactor instrumentation.
