You are receiving these forms because you have indicated to us that you are either a generator of hazardous waste and/or you own or operate a hazardous waste treatment, storage, or disposal (TSD) faciLity. Included in this package are the following: Instructions on which forms to complete. Form A - Identification Sheet and instructions. Form B - Self-Generated Hazardous Waste and Its Disposition and instructions. Form C - Hazardous Waste Received From Off-Site Handlers and Its Disposition and instructions. Form D - Hazardous Waste Minimization and instructions. Appendix Ihe instructions on the forms are self-explanatory. This report covers only hazardous waste that was generated, received, shipped,' treated, or disposed of during the calendar year 1985. The amount of waste that was in storage on-site at the beginning and at the end of 1985 is also to be reported. Please read all instructions carefully before attempting to complete the report. After filling out the forms, please review them for accuracy and completeness. Ail quantities of waste must be expressed in tons (2,000 lbs = 1 ton) to the third decimal place. Any other unit of measure will be unacceptable.
