The following data will be submitted prior to commencing work and/or during the progress of work to substantiate our billing invoicesi (1) Working agreements for crafts performing work. (2) Applicable percentage rate for payroll taxes, general liability insurance, workmen's compensation insurance. (3) Tools and equipment rental rates. Refractory Construetion, Inc.-owned. (4) Refractory Construction, Inc.-owned tools and/or equipment, rented to Coastal States Petrochemical Company, will be prepared on rental schedule and invoiced on either a daily, weekly or monthly basis, with Coastal State's representative approving all rental schedules. IH8URAMCE We will submit insurance certificates evidencing that we carry proper insurance; however, you may wish to review the limits and should you decide that you may want to increase these limits, this can be handled for very little increase in the insurance rates. Fee Our fee, applicable to the total payroll costs consisting of direct labor, payroll taxes, in surance and fringe benefits, including super visory personnel, shall be 17% to cover home office burden and overhead plus 8% to cover handling. IX.Consumables & Expendables All required consumable and/or expendable items at our cost plus 10% for handling. A representative list is enclosed to generally describe items considered under this category. X. Equipment Rental. Owned by Refractory Construction, Inc. All tools and equipment having a purchase price of $250.00 and above in accordance with published A.E.D. Rental Rates less 25%. A list of Refractory Construction, Inc.-owned equipment is attached. XI. Equipment Rental, Not Owned by Refractory Con struction, Inc. All required tools and/or equipment at cost plus 10% for handling. -4 - CED0006285 XII. Subcontracts All subcontracts as required at our cost plus 10% for handling, and awarded with the approval of Coastal States Petrochemical Company. XIII. Move-In and Move-Out All travel expense, freight, drayage and expediting charges at cost. XIV. Temporary Facilities & Utilities All required change houses, tool vans, field office and sanitary facilities at our cost. . The following data will be submitted prior to commencing work and/or during the progress of work to substantiate our billing invoices: (1) Working agreements for crafts performing work. (2) Applicable percentage rate for payroll taxes, general liability insurance, workmen's compensation insurance. (3) Tools and equipment rental rates. Refractory Construction, Inc.-owned. (4) Refractory Construction, Inc.-owned tools and/or equipment, rented to Coastal States Petrochemical Company, will be prepared on rental schedule and invoiced on either a daily, weekly or monthly basis, with Coastal State's representative approving all rental schedules. (5) Foreman's Daily Reports will be completed each day by our personnel and approved by Coastal State's representative. -5- CED0006286 (6) Payroll summary will be submitted, summarizing the Foreman's Daily Report. These weekly summaries will be attached to our invoices. (7) Shipping and receiving reports shall be prepared for material and supplies that are later to be billed to Refractory Construction, Inc. Coastal State's representative shall also approve these reports. C. INSURANCE We will submit insurance certificates evidencing that we carry proper insurance; however, you may wish to review the limits and should you decide that you may want to increase these limits, this can be handled for very little increase in the insurance rates. D. PERFORMANCE BOND AND/OR SPECIAL PERMITS A Performance Bond or special permits of any kind will be furnished, upon request, with the understanding that we are to be reimbursed any expense involved in ob taining such documents.
