SAFETY DEPARTMENT ANNUAL REPORT - 191+9
This annual report for the year 191+9 covers the following operations:
All plants of the Pittsburgh Plate Glass Company in the U. S. Southern Alkali Corporation plants at Corpus Christi, Texas
and Lake Charles, La.
Pittsburgh Corning Corporation plants at Port Allegany, Pa. and Sedalia, Mo.
One new organization was added tne first of the year; namely Moorestown, N. J. Corona Chemical plant of the Paint Division.
This gives us a total of 38 reporting units for the calendar year 191+9, in comparison with 36 organizations reporting for the full calendar year 191+8 and two months' operation of the Midvale Coal Mine for 191:8.
In comparison with the year 191+8 there was a decrease of approximately 6$ in the hours of exposure; a reduction of 23.1+$ in the frequency rate of accidents and a reduction of 53.8$ in the severity rate. Each of the four divisions--Glass, Chemical, Paint and Brush registered reductions in both frequency and severity rates. This enabled us to show a Frequency Rate of 6.61+, which is tne lowest rate shown
by the company since 1932, and a Severity Rate of .1+23, which is the lowest rate ever recorded by the company.
Seven organizations completed the year without a lost-time injury and will be issued a Certificate of Honor. We wisn to congratulate these organizations for their outstanding achievement.
Research and Laboratory, Creighton, Pa. completed their 7th consecutive no-accident year.
Gas Wells and Lines organization. Ford City, Pa. completed their 1+th consecutive no-accident year.
Springdale Paint plant completed their 2nd consecutive no-accident year.
Portland Paint plant completed their 2nd consecutive no accident year.
Shop #2, Ford City, Pa. completed their 2id consecutive no-accident year.
Los Angeles Paint plant completed their first no-accident year.
Suydam Paint plant completed their first no-accident year.

These plants will be issued Certificates of Merit in recognition of their achievement.
The severity rate for the year, not includinr a penalty of 6,000 days assessed for fatal injury, is .301 and 2$ organizations had records lower than this company rate for 19h9* Besides the seven organizations with clear records for the year, 16 additional organizations showed decreases in their severity rates over I9I48.
On the yearly comparative report, in the Group WAW plants. Works 06 was on top with a severity rate of .002 and a frequency rate of .6. In the Group frB" division Lake Charles, La. plant tops the list with a severity rate of .227 while New Martinsville, with only one lost-time injury, had the lowest frequency rate. In the Group "C" division, Research and Laboratory, completing their 7th con secutive no-accident year tops the list. Our congratulations to these organizations.
It is regrettable that we were not ai-le to complete the year without a fatality occurring to one of our employees. We had one fatal case which occurred in the Zanesville, Ohio Cement plant. In addition, there were nine permanent partial disability cases, seven of whicn were amputation of fingers and two were eye injuries, resulting in loss of industrial vision of the injured eye. These cases are listed on the special broadcast included in this report.
A program of physical examination of employees exposed to silica dust was set up in the plants of the Glass Division and tiie results of the initial examinations are, in most cases, favorable. Tie are continuing the Aluminum Dust Therapy Treatment of exposed employees and in 19k9, 166 employees received treat ment at Work5 #U, Ford City, Pa. Aluminum Dust 1herapy apparatus is being installed at Works #1, Creighton, Pa. for treatment of cases at that location.
A program of examination of lead-exposed employees was set up in two plants of the Paint Division and is operating satisfactorily in the prevention of lead absorption and lead intoxication of exposed employees.

Although major exposures to toxic materials have been eliminated or reduced in most plants, there still exists conditions which should be corrected, in order to minimize exposures to employees. The Industrial Ifygiene Foundation has made surveys of toxic and non-toxic dust and fumes in our plants and is continuing research on the toxic properties of materials which we handle and manufacture.
Safety Committee organizations have been installed in a number of our plants where they had not been functioning, and the results of their activities have been amply rewarded. Included in the Safety Ccmmittee organization is fire protection and housekeeping activities. Housekeeping has improved con siderably in most plants and our fire prevention activities have met with good results. However, we must realize that vigilance is necessary at all times in order to improve conditions and eliminate hazards which cause injuries or fire.
There has been increased use of moving picture films and slide films on subjects of safety and fire protection in the various plants. We have increased the number of films in our library and have an active circulation of films among the various organizations.
In starting the new year, we can feel that the efforts we have put forth during the past years have brought us good results in our accident-prevention campaign and although we did not achieve our ultimate goal in 19k9, the foundation we have laid should enable us to accomplish greater results in the years to come.
We are very thankful for the fine spirit of cooperation and interest manifested by all, and with increased activity in our safety program, we trust we will be able to submit a much more favorable report for the year 1950.

Employee was working in the lower level of the limestone quarry
and had spliced a length of electric cable running from the pump
house to the No. 92 shovel in the quarry. Apparently he was
about to connect the cable into the lighting system transformer
near the shovel when '-eceived a shock of U*0 volts, resulting
in his death-about 1:30 P.M. on September 29, 19U9*

Injured was taking down the hopper from above the mill alone instead
of using the chain and tackle. In so doing, the hopper slipped and
pinched right index finger between the hopper and the mill. Bone loss
tip of finger.

Employee had been called to plant for emergency work. He was lowering
a 300 gallon jelled plastic tank, using two one-ton double spur chain
hoists, which were fastened to the tank opposite one another. The
tank had been lowered to within six inches of the floor, using the
chainfalls alternately. As the tank was within six inches of the
floor, injured was checking to make sure that the thermo couple outlets
would not hit the floor and in so doing was holding onto the chainfall,
which had a little more slack in it, when either the other chainfall

had slipped or a link dropped, causing injury to the finger by the chain tightening and pinching finger against the rim of the tank. Complete avulsion of fat pad of distal phalanx of left index finger, denudation of bone and destruction of portion of tip of phalanx.

Service: 3 yrs. Employee was standing in #1 Evaporator holding a 1 inch drain line
up to Centrifix so it could be welded fast. He looked up at welder
and a drop of caustic fell from bottom of Centrifix into his eye.
The man was wearing goggles and the eye was irrigated immediately
at a nearby fountain. Third degree chemical burn of eye. Ulcer
on cornea| secondary infection. Loss of industrial vision of eye.

resulting in right hand passing over jointer blade. Amputation of
portion of second and fourth fingers right hand; laceration first
finger right hand; compound comminuted fracture of third finger
right hand.

Four fellow workers and injured were helping to install the right
steering shaft assembly in #2 Jumbo Brill Rig. In the process of
fitting the assembly to the bearing the end wedged.into outer
casting. The other end was held up by scaling bars by fellow
workers. The men holding the sealing bars were instructed to
release the load and when they did so the assembly broke loose
and dropped to the floor. The sharp edge on the end of the
assembly shaft came down on finger, Loss of end of right fourth
finger. Fingerre-amputated through the distal and proximal
phalanx.

Case No. 29317U Injured and fellow workers were moving 3A" plate 6' x 3" x 1*',
weight approximately 700 lbs., into #1 kiln. Injured was standing
on turntable and fellow worker was moving plate forward with a
spud wrench. The plate slipped off kiln apron and pinched finger
between plate and turntable. Compound fracture and partial ampu
tation of right fourth finger. Finger amputated below second joint.

into car and he reached down to pull on the bar and glass released,
catching his thumb between crossbar and the glass. Amputation left
