You are receiving these forms because you have indicated to us that you are either a generator of hazardous waste, a small quantity generator, and/or you own or operate a hazardous waste treatment, storage, or disposal facility. Included in this package are the following: - Instructions on which forms to complete. Form A - Identification Sheet and instructions - Form B - Self Generated Hazardous Waste and Its Disposition and instructions Form C - Hazardous Waste Received from Georgia Hazardous Waste Handlers and Its Disposition and instructions - Form D - Hazardous Waste Received from Out-of-State Hazardous Waste Handlers and instructions - Appendix The instructions on the forms are self-explanatory. This report covers only hazardous waste that was generated, treated, or disposed of during the calendar year 1983. The amount of waste that was in storage on-site at the beginning and at the end of 1983 is also to be reported. Please read all instructions carefully before attempting to complete the report. After filling out the forms, please review them for accuracy and completeness. All quantities of waste must be expressed in tons (2,000 lbs * 1 ton) to the third decimal place. Any other unit of measure will be unacceptable. The report must be completed and returned, no later than March 1, 1984, to: Industrial & Hazardous Waste Management.
