BAILEY, BUCK THOMERSON, JIM MILLIGAN, STEVE SUBJECT: Chlorine Scrubber Project - Pre-Start Up Safety Audit Followup A Pre-Start Up Safety Audit was held on June 12, 1991 for the Scrubber Project at the Chlorine Plant. This audit reviewed the safety, reactive chemical, environmental and operation aspects of the new system. The following are the concerns expressed during that meeting: 1. Have the vessel leak detection systems been added to the plant's C-9 Safety Checklist? 2. Does the Caustic Plant have an independent electrical feed system to the cell effluent pumps that would provide a back-up source of cell effluent to the new scrubber system? 3. Has extremely cold weather conditions been sufficiently studied as to the possibility that it may create a dry chlorine and titanium reaction under certain situations? 4. Have the procedures for the new system specifically addressed the caution that needs to be taken for welding/cutting on titanium and what action is to be taken if a titanium reaction does occur? 5. What is to be done with the temporary supports for the overflow of the T-15 (brine backwash tanks located near the new scrubbers) tanks? 6. It appears that a carbon steel valve is mounted on the drain for the pumptanks (PT-200A/B). What is to be done with this situation? 7. The re-enforcement pads on the project vessels need plugs installed. 8. AT-200B, one set of sparger gussets are cracked and need to be repaired. 9. AT-200B, a set of ladder supports appear to cracked. 10. The scrubber effluent recycle line (from T-l) drain DO A 023422 CONFIDENTIAL valve handle may be in the walkway wh n closed. All of the abov concerns will be addressed prior to start-up. I would like to thank all those who participated in this audit, I feel it was a meaningful exercise.
