REYNOLDS METALS SHERUIN PLANT JUNE 1992 MONTHLY REPORT This report is inclusive of Asbestos removal activities from May 31, 1992 until June 27, 1992. There were three contractors on site, TGI Stephens, Industrial Specialists Incorporated, and Falcon Associates. TGI Stephens was issued an exception report qr 6/16/92. The type of exception was non-compliance with state/federal asbestos regulations. The classification was serious and citable by the regulatory agency. The action that was required by NWE was for the contractor to stop work, submit a plan to correct the deficiency within 4 hours. Please see the attached Exception Report form. This month there was one reportable injury from TGI Stephens. The inj'ury occurred at the #12 Washer. The employee that was inj'ured reported a skin burn. Communications between Reynolds Metals, Northwest Envirocon, Inc. and the three contractors seems to be excellent at this time. July 10, 1992 B.000651 i If N REYNOLDS METALS - SHERWIN PLANT ' WEEKLY REPORT WEEK ENDING JUNE 6, 1992 This week there were three capital projects, one maintenance project, and four small scale/short duration project worked this week. There were three contractors on site to perform the work TGI Stephens, Industrial Specialists, Inc. and Falcon Associates. OLD MEDICAL BUILDING. Industrial Specialists, Inc. was the contractor to perform the work on this capital project. I.S.I. has nine workers and three supervisors on the project. I instructed them to fix the holes in the flaps and leave the flaps down. One PDM machine was adequate for the manometer to read .02 inches of water. On 06/01/92 I discussed with Mr. Rodriguez of I.S.I. that the mastic had not been completely removed from the two rooms and that this should be done before the pipe and the duct is done. Mr. Rodriguez informed me they would place poly over the floors in order to set the ladders down I.S.I. did ceiling scrapes and glove bagging. They cut down duct and pipe double wrapping them both. I did final cleaning on the ceiling scrape on 6/02/92. During the inspection I showed Mr. Ordonez of I.S.I. where RACM was on the beams and ceiling. The enclosure did not pass. Paul Matula of RMC came by and I informed him that the ceiling scrape was not completed and the contractors were standing on planks on the hand rail of the rolling scaffold. Mr. Matula told I.S.I. to change the scaffold. C STATION DESUPER HEATER. TGI Stephens was the contractor to perform the work on this small scale/short duration project. The contractor had three men and a supervisor in tyvecks and PAFR's on the job site. There were three glove bags removed. I showed Mr. Arriaga of TGI Stephens three places to tape where the seam was not taped solid. I called Mr. Smith of TGI Stephens and informed him about his crew not taping up the glove bags all the way. This project had high fiber counts of 1.16 F/CC. #4 BOILER. Industrial Specialists, Inc. was the contractor to perform the work on this maintenance project. Mr. Rodriguez removed his crew from the 84 Boiler project to work in the Old Medical Building. Falcon Associates was the contractor to work this capital project. The contractor arrived on 6/01/92. but the paperwork was incomplete. I informed them they could unload, but could not proceed beyond that point until the paperwork was complete. #12 WASHER. TGI Stephens was the contractor to perform the work on this capital project. On 6/02/92 it was still raining and the contractor was unable to work on the enclosure. I performed the visual on 6/03/92, On 6/04/92 the contractor finished up an area by the West Over flow that had to be taped. I requested another row of tape be added. I inspected the NW side and Mr. Gonzales did the SE side inspection. TGI Stephens had 18 men on site. The manometer was .05 inches of water. There were 9 POM machines with two blowing cool air. Abatement began on the West side. BUILDING 111 STEAM PIPING. Industrial Specialists, Inc. was the contractor to perform the work on this small scale/short duration project. There were three lines that were wrapped, cut, and disposed of. 5-7 DISCHARGE LINE. Industrial Specialists, Inc. was the contractor to perform the work on this small scale/short duration project. The contractor performed glove bags to abate the discharge line on overtime. REYNOLDS METALS - SHERWIN PLANT WEEKLY REPORT WEEK ENDING JUNE 13, 1992 This week there were three contractors on site, Falcon Associates, Industrial Specialists, Inc., and TGI Stephens. There were two capital projects and one maintenance project performed this week. 84 BOILER. Industrial Specialists, Inc. was the contractor to perform the work on this maintenance project. Mr. Rodriguez had his crew working on the pit cleaning up the oil inside of the pit. On 6/11/92 at 10:00 I.S.I. was performing final cleaning on the North Side enclosure. After the clearance passed, I.S.I. built the enclosure on the South side. OLD MEDICAL BUILDING. Industrial Specialists, Inc. was the contractor to perform the work on this capital project. A visual was done on the ceiling and it was very clean. The floor was not clean and the area had to be gone back over . On 6/9/92 I checked the sewing room and I.S.I. had not started glove bagging the elbows or the RACM from the pipe. The contractor had been abating RACM Mag block, air cell, vat, and mastic from the sewing room. The X-ray room had four areas that needed re-cleaning. I checked the containment floor which was 3/4 covered with 2-6 inches of liquid caustic. I passed the visual and informed I.S.I. they could encapsulate. The caustic was cleaned out after the clearance. Clearance passed at .003 F/CC. I informed Mr. Rodriguez of the four elbows in the sewing room that had to be abated. At 8:20 I.S.I. had three workers which began to abate the elbows. The Hepa Vac was not turned on. The workers were in 1/2 masks and tyvecks glove bagging. 812 Washer. TGI Stephens was the contractor to perform the work on this on going capital project. Mr. Paul Matula called and asked if the contractor could abate a pipe on top of the 8 12 Washer open air. I informed Mr. Matula that the tar paper was RACM. On 6/10/92 TGI Stephens was prepping the East side of the Washer and was abating oh the 4th level inside. There were three contractors on site to perform the work. Industrial Specialists, Incorporated, TGI Stephens, and Falcon Associates. #6 ESP. Falcon Associates was the contractor performing the work on this ongoing Capital project. The contractor had a crew of eleven men hanging poly. They were also working on the decon. I performed a pre-abatement visual on 6/17. They were prepping the bottom of the enclosure along with patching and taping the weak areas in the poly. Mr. Paul Matula of RMC asked me if Falcon could store all the bags up and around the #6 ESP. I said yes as long as there is poly under the bags and danger tape around the bag storage area. Mr. Ernesto Gonzales of NWE did the pre-abatement visual on 6/17/92. There were numerous holes and the roof was down. The enclosure was not ready for the inspection. On 6/18 Mr. Whitman had not placed the power panel topside. The manometers did not work, so the inspection regained pending. #12 WASHER. TGI Stephens was the contractor to perform the work on this ongoing capital project. The crew was hanging poly and performing abatement. The contractor was abating on the bottom level and began to detail the top level. Roger Fay took PDM air samples and the readings were high. I told Mr. Tommy Smith of TGI Stephens that the four PDM machines on the top of # 12 Washer needed to be shut down 1 and the HEPA's changed, wiped out and then restarted. On 6/18/92 TGI Stephens had seventeen abatement workers performing final cleaning and working on the east side enclosure . #8 ESP Industrial Soecialists, Inc. was the contractor to perform the work or. this maintenance project. This week consisted of building framework and hanging poly with a crew of seven men. 3-1 FLASH TANK. Industrial Specialists, Inc. was the contractor to perform the work on this encapsulation project. The job consisted of two men applying fab cloth and mastic. page 1 of 2 OLD MEDICAL BUILDING. Industrial Specialists, Inc. was the 1 contractor to perform the work on this ongoing capital project. REYNOLOS METALS - SHERWIN PLANT m on the Air* Conditioning and the duct on the outside of the Old Medical Building. The samples tested positive for RACM. Mr. Rodriguez and I checked the duct and an estimation was made by Mr. Rodriguez that he could cut the duct in a couple of places, but he needed a cherry picker. I checked the mobile decon trailer that belonged to I.S.I. and found it filthy. I picked up a 1/2 mask respirator with filters from the clean room. It was not in a bag and the filters were not taped. #4 BOILER PENTHOUSE. Industrial Specialists, Inc. was the contractor to perform the work on this ongoing maintenance project. On 6/16/92 a pre-abatement visual was done on the decon and bag out. The work area itself is inside the boiler and does not require poly. The contractor patched and taped two areas in the decon. The manometer read .,02 .inches of water. I pointed out to George Acosta of I.S.I. that the waste water pump was in front of the filters and it could get debris and RACM in it that may contaminate or destroy the pump. On 6/17 I performed the post abatement visual and found four areas that required additional cleaning. 1 & 2 CLEAR FILTRATE STATION HEADER. Industrial Specialists, Inc. was the contractor to perform the work on this small scale/short duration project. I.S.I. performed glove bags on two sections of 14" pipe. ORANGE S HEADERS, Industrial Specialists, Inc. was the contractor to remove the tar paper from the header at the #5 Filter Feed Tank. This was done on overtime after RMC removed the pipe and lowered it to the ground. Only the weather coat was RACM. PROJECT ADMINISTRATOR REYNOLDS METAL - SHERWIN PLANT WEEKLY REPORT B.000657 REYNOLDS METALS - SHERWIN PLANT WEEK ENDING JUNE 27, 1992 This week there were two capital projects, one maintenance project worked this week. There were three contractors on site this week. Industrial Specialists, Inc, TGI Stephens, and Falcon Associates. I] II II u II II n #12 WASHER. This was an on going capital project. TGI Stephens had six men repairing the enclosure. The contractor had a great deal of workers quit because the enclosure is extremely hot. The contractors crew was detailing the West Side using electric hammers. On 6/25/92 a PDM machine failed. Mr. Tommy Smith of TGI Stephens sealed both of the ends and shut the machine down. He felt that the reason the machine failed was because fibers were blowing across the top of # 12 Washer. ' #8 KILN DUST HOPPER. Industrial Specialists, Inc. was the contractor to perform the work on this ongoing maintenance project. The contractor was setting up the PDM machines. Mr. Rodriguez informed Mr. Hector De La Garza the #8 ESP had a large opening through the roof that was to be abated and they had 3 PDM machines running and could not draw in .02 inches of water of the manometer . A decision was made to poly the roof and when .02 inches of water was achieved the Hopper and Cyclone could be abated. #6 ESP. Falcon Associates was the contractor performing the work on this ongoing capital project. The contractor had continuing problems with the manometer. Mr. Whitman called his office for more negative air machines and a new manometer. The roof came down again and it had to be repaired. Abatement began using the wet method of abatement. There were numerous olaces on the bottom and sides of the enclosure that leaked small amounts of contaminated water. Falcon fixed the prcolem by taping the leaks and Hepa Vacuuming the water. I talked to Mr. Matula about the heat at 86 ESP caused by the 5 & 7 ESP running. The heat may cause the walls of tne enclosure to fall down.
