The facility was inspected to ensure compliance with standards for hazardous waste generators and universal waste management (40 C.F.R. Part 262 through 273) and used oil management (40 C.F.R. I presented my credentials to Ms. Kalinka Guffens and informed her that this was an EPA RCRA inspection. The table above presents all the inspection opening and closing conference participants. Facility/Site Description The Fairbanks Memorial Hospital is a community hospital located in the City of Fairbanks, AK. The Hospital is owned and operated by Foundation Health Partners. SQG verified by Ms. Guffens Number of employees 1,500 Process Description Ms. Guffens told me that the hospital generates the following wastes: Trace Chemo Waste - accumulated in yellow bags/containers. Medical Waste - accumulated in red bags/containers. Hazardous Waste Pharmaceuticals (HWPharms) - pharmaceutical waste from the pharmacy, floors, and surgery. Lab Waste - flammable liquids (alcohol/xylene mixture) - managed as hazardous waste. Non-hazardous and non-regulated wastes. Waste batteries (lithium, Ni-cad, lead-acid) and waste lamps (4 ft. and 8 ft.) managed as universal waste. P-listed Waste - cartridges for nicotine inhalers managed as hazardous waste. Used oil Waste Aerosols - managed as hazardous waste. According to Ms. Guffens, the facility does not puncture aerosol cans. According to Ms. Guffens, the following four employees assist with waste handling: Alvin Brown - handles regulated medical and chemo waste John Vantuil - handles regulated medical and chemo waste Connie Moore - handles universal waste William Magetich - According to Ms. Guffens, Mr. Magetich is a new supervisor that will be starting soon. She told me that he will also be handling universal waste. Ms. Guffens told me that there were approximately 35 satellite accumulation areas (SAA) and two central accumulation areas (Flammable Shed and Compactor Room). She said the facility manages their waste through Clean Earth Specialty Waste Solutions. During the inspection of the facility, I was accompanied by Ms. Guffens. During the inspection I looked at the facility's processes, in addition to hazardous waste management practices, generation points, and accumulation areas. I looked for wastes that facility representatives had not yet identified or designated as hazardous. I also observed the facility's universal waste and used oil management. The contents of the container were also verified as HWPharms by Ms. Guffens. The container was not closed and was also not dated. Ms. Guffens closed the container of HWPharms during the inspection. While at the same SAA, I saw another container that Ms. Guffens said was a satellite accumulation container for the accumulation of P-listed hazardous waste (cartridges for nicotine inhalers). At the time of the inspection, the satellite accumulation container was not labeled with the words "Hazardous Waste" and was not marked with a hazard indicator. One container of four-foot waste lamps was labeled with a universal waste label and dated 6/13/2016, showing that it had been in accumulation for greater than one year. Another container of four-foot waste lamps had no universal waste labeling and no date. The container had a piece of paper taped to it that contained the words, "This drum is only for the disposal of Fluorescent lamps that are a full 4 feet long - nothing shorter." Following the inspection, Ms. Guffens sent me an email, dated 5/13/22, with a photo showing that the container was labeled with a universal waste label. At the time of the inspection the paper taped to the container was covering the universal waste label. In Ms. Guffens email, she stated - "Here are pictures of the light tubes in 008ST. The Universal waste sticker was behind the A4 paper. The second picture is the same drum just a close up of the sticker with a date of 3/5/18." One of the two cages contained two "CleanPak" cubic yard sacks that were on pallets. One CleanPak held non-regulated waste and the other held HWPHARMS. 8 of 54 FAIRBANKS MEMORIAL HOSPITAL Inspection Date(s): 05/11/2022 - 05/12/2022 The CleanPak, containing HWPHARMS had the inner orange bag closed, but the actual CleanPak was not closed. The CleanPak was marked with a hazardous waste label and dated 4/24/22. It was also marked with DOT class 3 and class 6 hazard indicators. The CleanPak of HWPHARMS was not labeled with the word "HWPHARMS." The date on the CleanPak is the date that the facility is using as the date the non-creditable HWPharms first becomes waste. However, the date they are using is the date that the first of the smaller accumulation containers is added to the CleanPak and not the date that the non-creditable HWPharms were first placed into the smaller containers that are located throughout the hospital. According to the accumulation start dates, the last two containers of waste, listed above, had been in accumulation for greater than 270 days. I also saw a container of Nalco 2840, that Ms. Guffens said had not yet been designated. She said it was received last month, from Planned Operations (Maintenance). Ms. Guffens provided a copy of the Safety Data Sheet (SDS) for Nalco 2840 during the inspection. A cursory review of the SDS (Included in the document log) indicated that the waste was toxic and corrosive.
