Scope of Work: IWSS will provide supervision, labor, equipment, material, and insurance to perform the demolition of the above referenced project within all federal, state and local regulations. IWSS will perform the demolition of the sinter plant, sinter plant bag house, dross reverb bag house, blast furnace, dross reverb, reverb, reverb brick flue, roaster building, zinc plant bag house, zinc kiln area, cd plant with the understanding that IWSS is responsible for the removal of scrap iron and copper wire and it will take ownership of this material. IWSS will leave concrete structures behind for removal at a later date. Copper Slabs located through out the structures and primarily at the dross reverb and reverb will be left for salvage later by Asarco. Following an extensive project walk through, it was determined that many of the plants active lines run through areas that are slated for demolition. These lines will need to be relocated or protected in order for demolition work to proceed in a cost effective manner. Enclosed you will find a brochure that lists the work that IWSS performs in the environmental field. Our abilities reach beyond the demolition field. During my project walk through on February 22, 1996, we discussed the lead hazard plant wide and the facilities available for the employees and contractors to protect them from the dangers of lead. In most plant demolition projects, the demolition contractor is on his own to protect its employees from the hazards of lead. The use of the wash facilities, lunch room and laundry service is beneficial to the contractor in its fight to protect its employees from overexposure to lead. Our proposal includes the cost of power washing loads of scrap prior to leaving the demolition site and exiting the plant. The present plan for removal of scrap from the plant is using the existing rail service. Prior to loading scrap into rail cars, the scrap will be cleaned, preventing the migration of lead dust from the plant. Coordination between demolition supervisor and plant maintenance personnel will be important for protection of Asarco employees, and IWSS demolition crews. The relocation and cutting of utilities prior to demolition is the first line in defense in preventing personnel injury and plant disruption. Houston, Binghamton, NY ASARCO ELP 0010378 Considering the OSHA Lead Standard alert, that exposure to lead, lead dust or lead materials possibly constitutes a potential health risk to IWS and IWSS employees, the following is an outline of IWS' Lead Program and Lead Awareness Training Program. OSHA finds that risks associated with lead are magnified by improper and uninformed manners in which lead (materials) are handled. Improper design, implementation or inspection of lead projects creates unnecessary health and safety hazards which are detrimental to the employees and citizens involved with the work (neighborhoods). IWS, therefore, finds it to be its purpose and policy to reduce lead related hazards by encouraging proper training of perspns employed to design, implement or inspect lead projects and those who supervise or employ them by requiring proper certification and training before any handling of lead materials and enforce safety and health standards regulating the conduct of those projects unvolving lead or lead materials. This Lead Standard will provide IWS' construction workers with the same level of protection as general industry workers. Benefits of this program will consist of a reduction in health related cases of over-exposure to lead. This Lead Standard applies to all construction workers exposed to lead. It includes metallic lead, all inorganic lead compounds and organic lead soaps. Type of Work; Workers involved in the following type of work are affected - demolition or salvage of structures, removal of lead materials, construction, alteration, repair or renovations of structures, substrates or portions thereof, installation, cleanup, transportation, disposal, storage or containment of materials and maintenance opertaions. Lead Related Tasks; OSHA considers lead related tasks to include - abrasive blasting, welding, cutting and burning on steel structure, lead burning or torch melting or fusing of lead or celloyed lead to another lead object, manual scraping and sanding associated with paint removal, manual demolition of structures coated with lead ' based paint, heat gun application to melt lead paint, and cleanup activities where dry expendable abrasives are used. Permissible Exposure Limit; IWS will assure that no employee is exposed to lead concentrations in excess of the PEL of 50 micrograms of lead per cubic meter of air (50ug/m3), the same as general industry standards. The limit is an eight-hour average for any work day, known as a Time Weighted Average (TWA). If respirators are worn, employees need use them only for a period of time that will produce an average exposure below the limit. Exposure Assessment; IWS will obtain an objective data on the extent of lead at the jobsite or monitor lead exposure. Monitoring includes taking air samples in the worker's breathing zone. If the exposure is at or above the action level, IWS will continue to monitor on a prescribed schedule. At the "action level" of 30ug/m3, IWS will initiate compliance activities. Methods of Compliance; IWS will initiate engineering and work practice controls to reduce exposure below the permissible limit. If this is not possible, respiratory protection is required. Written compliance plans are then to be developed and implemented, if respiratory protection is not provided. Respiratory Protection; The rule requires that respirators be used whenever the concentration of lead is at or above the permissible exposure limit, if engineering and work practice controls are not sufficient or whenever an employee requires a respirator. Employers must provide powered or air purifying respirators to its employees. It is the contention of IWS that the established Respirator Program will be followed considering proper selection, use, cleaning and maintenance of respirators worn for lead procedures.- Protective Clothing and Equipment; IWS will provide, at no cost to the employee, the protective clothing and equipment appropriate for the hazard for lead. Clean work clothing will be provided at least weekly in high risk lead exposure areas. ASARCO ELP 0010380 Housekeeping; All work surfaces will be maintained free of accumulation of lead dust by washing or vacuuming the areas. Hygiene Facilities; IWS will provide adequate shower facilities, clean areas for changing clothes, and eating areas for employees who are exposed above the limit of the PEL (50ug/m3). Smoking, eating and applying cosmetics are prohibited in lead areas. Medical Surveillance; Two phases of medical surveillance are required; (1) Employees exposed above the limit are entitled to initial medical surveillance, by which IWS is required to provide blood sampling and followup checks under the super vision of a licensed physician, and (2) If the employee's blood level exceeds 40 micrograms per decileter for more than 30 days a year, a full medical surveillance program, including annual medical exams, is required. Medical Removal Program: If an employee's blood is above this limit, IWS will remove the employee from work associated with lead exposure. Employee Information and Training; IWS will provide informations and,-training programs for all employees exposed to lead at or above the action level. Signage; Warning signs will be posted in each work area where an employee(s) can be exposed to lead that exceeds the permissible exposure limit. Recordkeeping; IWS will maintain an accurate biological and environmental monitoring of records of employees exposures to potentially toxic materials and provide employees access to the records. Integrated Waste Special Services, Inc. By integrating environmental contracting, industrial cleaning, and demolition/dismantlement services in the way it has, Integrated is fully able to single-source most projects on which it bids. As shown in the subsections that follow, this means that not only can Integrated remove hazardous waste during a plant decommissioning project, it also can flush/clean piping, vessels, and in-plant sewer lines; decontaminate facility structures and equipment; remove and/or encapsulate asbestoscontaining material; and provide partial or complete demolition/dismantlement services utilizing inhouse resources. 2.1 ENVIRONMENTAL CONTRACTING Integrated offers a complete line of environmental contracting services including; * Site Utility and Groundwater Treatment System Installation; Waste (Hazardous and Non-Hazardous) Containment, Identification, and Disposal; Site Remediation; General and Site Contracting; and , Asbestos Abatement and Encapsulation. 2.1.1 Site Utilities & Groundwater Treatment Systems Integrated's Environmental Contracting Division has extensive experience in the removal, relocation, and/or reinstallation of site utilities, and the installation of monitoring wells, groundwater pump and treat systems, and oil/water separator units at sites throughout the United States. Groundwater remediation and management services provided by integrated's Environmental Con tracting Division include; Monitoring well installation, Groundwater sampling, and Contaminant recovery, removal,and treatment. 2.1.2 Waste Containment, Identification & Disposal Integrated provides a full scope of solid and liquid, drummed and bulk waste management services in support of its environmental contracting and industrial cleaning projects. Hazardous and nonhazardous waste containment, identification, and disposal services provided by Integrated include: sampling, characterization, analyses, consolidation (as appropriate), DOT-approved containerization, packaging/labeling, manifesting, loading, transportation, and treatment/disposal. These services apply not only to waste generated as a result of remedial action or industrial cleaning operations, but also to known and unknown waste found in buried or stored drums, containers, piping, and vessels located at a client's facility or project site. Additional services provided in association with these types of projects include locating, excavating (if buried), assessing, and overpacking (as necessary) drums prior to relocation to a temporary staging area for characterization, consolidation, and disposal. 2.1.3 Remedial Site Services Integrated's Environmental Contracting Division offers a full range of hazardous and non-hazardous remedial site services including, but not limited to: Contaminated soils delineation, characterization, excavation, transportation, and disposal; *- Above- and underground storage tank clean-out and closure; Landfill and surface impoundment closure; Integrated^ Waste Special Services, Inc. iwsssoanvifi.se ASARCO ELP 0010394. >- Product line decommissioning and industrial plant decontamination; Removal and proper disposal of PCB-contaminated electrical equipment; and On-site remedial system construction for treatment of contaminated groundwater and soil. Integrated also has the technical and physical resources needed to safely and effectively implement the full spectrum of engineered landfill/lagoon closure programs and site remediation plans. 2.1.4 General & Site Contracting As a single-source contractor, Integrated offers the following general and site construction services to round out its Environmental Contracting program: Temporary access/haul roadconstruction; Security fence installation; Support facility construction; Structural demolition/dismantling; Earthworks (i.e., excavation, backfill, regrading, etc.); Underground utilities (i.e., sanitary, storm, water); Parking lot, roadway, bridge, and concrete curb construction; and Landscaping. 2.1.5 Asbestos & Lead Abatement Integrated's Asbestos Abatement Group operates under the company's Environmental Contracting Division and specializes in providing a full range of heavy industrial and commercial asbestos services including: Facility/building assessments and surveys; Bulk sample collection and analysis; Third-party air sampling collection and analysis; Removal, encapsulation, and enclosure of asbestos containing materials; * Transportation and disposal; * Complete chain-of-custody for all operations; Re-insulation; and/or Complete documentation for all phases of the abatement project. Integrated's Asbestos Abatement Group is fully licensed and permitted to perform all of the above services. For example, the Group holds a license by GPAC, Inc. for use of a negative pressure containment method for asbestos removal. This negative pressure containment method is required undercurrent United States Environmental Protection Agency (USEPA), NESHAPS, and New York State Labor Law Code Rule 56 regulations. Integrated also carries a New York State Department of Environmental Conservation (NYSDEC) Transporter Permit Part 364 to haul asbestos waste to certified and fully licensed landfills in New York, Pennsylvania, and Ohio. A feet of roll-off con tainers, lugger boxes, walking floor trailers, dump trailers, and closed body box vans (see Table 1-1) enable Integrated to provide virtually unlimited transportation and disposal services. Integrated often coordinates asbestos removal with other site services, such as lead paint removal, to maximize manpower utilization and minimize remediation costs. Lead abatement services provided by Integrated include: Inspection, testing, and documentation of results; Remediation alternatives and costanalysis; Lead paint removal and proper hazardous waste disposal; Encapsulation with USEPA recommended material application; and/or Water testing and treatment. Integrated's team of experienced asbestos and lead abatement experts work closely with its client's representatives (i.e., facility production personnel, plant management, etc.) to identify contaminants within their property and prepare cost-effective, safe asbestos and lead management plans for the near and long-term future of the facility, be it industrial, commercial, or residential. All of Integrated's contaminant abatement/management programs utilize state-of-the-art equipment, technology, and procedures. 2.2 INDUSTRIAL CLEANING Integrated has over 25 years of experience in performing industrial cleaning and maintenance services for a nationwide client base which includes steel manufacturers, automobile companies, and chemical and bulk waste processors. Integrated2 Waste Special Services, Inc. iwsssoq rvi/i-ss ASARCO ELP 0010396 With its fleet of modern equipment (i.e., Guzzler vacuum trucks, N.L.B. water blasters, ram rodder equipment, and sewer cleaning video inspection units), Integrated's Industrial Cleaning Division can provide a full slate of industrial services for the cleaning of storage tanks, elevator pits, sumps, lagoons, flyash hoppers, boiler tubes, cranes, condensers, tube bundles, and coal bins. Integrated's Industrial Cleaning Division also is available on a 24 hour per day, 365 day per year basis to rapidly respond to and clean up solid and liquid material spills (call 1-800-850-4977) Industrial cleaning services offered by Integrated include: 500 PSI to 35,000 PSI high-powered water blasters capable of accomplishing a wide variety of industrial cleaning tasks; Wet and dry vacuum trucks with 2,500 to 6,000 CFM vacuum blowers and 14 to 18 cubic yard capacities which can handle waste from flyash hoppers to tanks and roadside spills; Removal, transportation, and disposal of sludge, liquids, and solid industrial residue and waste; Line purging, sewer flushing, video inspection, and other specialty services; Emergency response services (1-800-850-4977); Design and implementation of continuous cleaning for production-type processes; and Site-specific industrial cleaning equipment design for specially processors. In addition to addressing current problems, Integrated's Industrial Cleaning Division works with its industrial clients to develop long-term procedures to manage future environmental challenges in a controlled and cost-effective manner through the development of System Maintenance and Emergency Preparedness and Response Plans. 2.2.1 Sewer Flushing & Maintenance Integrated's Industrial Cleaning Division maintains and is experienced in the operation of specialty sewer flushing and maintenance equipment including video camera units for remote sewer inspection, and sewer flushers capable of generating 2,500 PSI at 80 gallons-per-minute and accessing two to 52 inch diameter drains. Each vacuum truck is equipped with a four inch sludge discharge pump. 2.2.3 High Pressure Water Blasting Integrated can provide 500 to 35,000 PSI water blasters to surficially decontaminate/clean sewer lines, process equipment, boiler tubes, tube bundles, condensers, structural components, etc. When required, the effectiveness of this service can be verified through remote video inspection by Integrated's Industrial Cleaning Division or confirmatory wipe sampling by Integrated's Environmental Contracting Division. 2.2.4 Emergency Response & Plant Shut-Down Services Integrated provides 24-hour emergency response service to its industrial clients. Depending upon location, Integrated's trained Environmental Contracting and Industrial Cleaning personnel can be on-site within two to four hours to safely contain the release and initiate environmentally-compliant clean-up operations. Following initial response, Integrated can develop and implement full-scale Remedial Action Plans and assist clients in evaluating the effectiveness of their Emergency Response Programs. 2.3 DEMOLITION & DISMANTLEMENT A member of the National Association of Demolition Contractors, Integrated's Demolition and Dismantlement Division has successfully completed scores of projects including, but not limited to, the razing of plant facilities, municipal buildings, bridges, and production-related industrial structures. By integrating its Environmental Contracting, Industrial Cleaning, and Demolition/Dismantlement Divisions, Integrated is able to both assist in the design of and fully implement a wide variety of facility decontamination and demoiition/dismantlement project plans utilizing in-house resources. Integrated's operational plans take into consideration that most industrial clients cannot fully shut down production operations to allow for removal or selective demolition. By calling upon the shared experiences of all its operating groups, Integrated is able to effectively design and implement procedures which meet its client's specific operating needs and scheduling concerns. Integrated^ Waste SpecialServices, Inc. iwsssoqrvw.ss ASARCO ELP 0010398 Integrated's Demolition'and Dismantlement Division was significantly enhanced in 1993 by the acquisition of US Dismantlement Corporation, a nationally recognized and respected demolition contractor whose clients include General Motors Corporation, Ford Motor Company, Chrysler Corporation, Stein & Company, Morse Diesel International, Fluor Daniel, Home Depot, Lasalle Construction, and Komatsu-Dresser. 2.3.1 Plant Dismantlement & Engineered, Selective, and Interior Demolition Services provided by Integrated's Demolition and Dismantlement Division include: Implosion and/or controlled collapse of structures and facilities; * Conventional demolition; > Interior demolition (gutting); Specialized and/or engineered dismantling of complete or partial production facilities; Design, lay-out, and budgeting for future and/or anticipated projects; Emergency response services for fire, explosions, and natural disasters; Ferrous and non-ferrous metal processing, removal, transportation, and re-sale; and Equipment rigging, moving, and relocation. 2.3.2 Investment Recovery & Equipment Resale As an extension of its demolition/dismantlement service, Integrated helps its industrial clients recover investment from non-revenue producing inventory or production by-pass. Integrated's Investment Recovery and By-Pass Resale Product Group can provide: Identification and containerization of by-pass and excess raw materials; Computerized by product matching services which identify end uses of excess materials; Investment recovery of out-dated equipment and production facilities; and Quality control and re-design of production facilities to reduce and/or modify by-pass material so that it can be reused by other manufacturers. Occasionally, when there are no other alternatives for re-use, Integrated, through its parent (IWSI), has direct access to an extensive network of company-owned and third-party hauling, disposal, and treatment facilities. When off-site treatment/disposal is required Integrated's Environmental Con tracting Division steps in to identify, manifest, transport, and arrange for regulatory compliant treatment or disposal of the hazardous and/or non-hazardous waste streams (see Section 2.1). Integrated2^ Waste Special Services, Inc. iwsssoa.uvm.# ASARCO ELP 0010399 SECTION 3.0 Corporate Health & Safety All of Integrated's service divisions and groups operate under the same Comprehensive Loss Prevention Program which requires the: Issuance of Integrated's Corporate Health and Safety Manual to each supervisor who has an operations area; Performance of job safety analyses to identify and provide controls for the anticipated hazards expected to be encountered at each project site; Completion of Safety Awareness Contact Records by supervisors to document employee safety training and performance; Provision of general and job-specific training and medical monitoring; and Performance of weekly and monthly facility and job-site inspections by the Safety Department and Administrative Supervisors to continually monitor both the workplace and the work practices is use. As indicated, Integrated's Corporate Health and Safety Manual is issued by control number to each supervisor who has an operations area, be it within the Environmental Contracting, Industrial Cleaning, or Demolition/Dismantlement Divisions. This manual contains copies of the company's safety policies and general safety rules that apply to all groups. It also contains a SOP section which contains copies of all Standard Operating Procedures that pertain to the Division or Group in which the manual has been issued. This section is kept updated as new needs are identified and new practices developed. Integrated's Corporate Health and Safety Manual also contains a Job Safety Analyses requirement which sets forth the procedures to be used to identify anticipated job-specific hazards (chemical, physical, and/or biological) and develop appropriate work practices and controls to abate these hazards. Another important feature of Integrated's Corporate Health and Safety Manual are the Safety Awareness Contact Records provided for use by the company's operational supervisors. This document provides a record sheet for each person under the supervisor's authority on which all safety meetings are recorded. It also provides a staged discipline procedure for the supervisor to use in upgrading the employee's overall safety awareness. The Safety Awareness Contact Records are reviewed monthly by Integrated's Safety Department and Administrative Management Team to ensure their proper use and quality of maintenance. They are audited yearly to evaluate the overall ranking of each employee's safety record. This ranking is discussed with employees during their annual review and is reflected in job promotion and compensation decisions. As part its Comprehensive Loss Prevention Program, Integrated's Safely Department and Administrative Management Team conduct weekly and monthly facility and job-site inspections to monitor both the workplace and the work practices being employed by the company and/or any of its subcontractors. All of these inspections are rated on a point system and, for each 90-day cycle, the supervisor with the highest points receives a Superior Performance Award for their safety effort. Integrated's upper management believes that the above Comprehensive Loss Prevention Program combined with constant, hands-on management of the company's safety and general supervision personnel keeps its safety awareness at the highest level. Prior to mobilization. Integrated designed and implemented a site-specific Health and Safety program which incorporated all phases of the contract work and provided the precautions necessary to protect both Integrated site employees and Carborundum plant personnel. Integrated initiated site operations with the removal of building materials contaminated with asbestos and radium. PCB contaminated electrical equipment (i.e., circuit breakers, fused cutouts, and double throw switches) also were removed for disposal during this project phase. Once all above-ground contamination was removed, Integrated's asbestos abatement division was mobilized to the site to conduct Phase II, Asbestos Abatement, operations. Over 6,000 cubic yards of asbestos containing materials were removed from the facility including boiler breaching, pipe lagging, floor tile, flashing, and roofing materials. NOTE: The abatement involved removal of ACM from exposed heights of greater than 30 feet. Phase III, Demolition, was conducted by Integrated's demolition division and included the dismantlement of over 18 industrial structures. This project phase was highlighted by two implosions-one of which felled a one million cubic foot, five-story abrasives factory building. The final implosion razed two, 250 foot stacks and two, five-story production buildings. Other structures were dismantled utilizing mechanical shearing technology. The project was completed on schedule and within budget to the complete satisfaction of the client. Integrated____________ Waste Special Services, Inc. iwsssoo Rvi/i-os ASARCO ELP 0010405 PROJECT DESCRIPTION PROJECT: Emergency PCB Spill Clean-Up & Associated Demolition Buffalo, New York CLIENT: Independent Cement Corporation CONTACT: Tim Harper, Plant Manager (716) 825-7084 VALUE: 54,500,000 DATES: September 1992 - February 1993 DESCRIPTION: Integrated provided emergency response services in support of a PCB spill clean-up effort at Independent Cement Corporation's Buffalo, New York Facility. Operations were conducted by Integrated under the direction of the New York State Department of Environmental Conservation (NYSDEC) and entailed exploratory excavation, contaminant delineation, contaminated soils excavation and disposal, asbestos abatement, and structural demolition. Exploratory excavation conducted by Integrated uncovered large concentrations of PCB-contaminated liquid at the site. To prevent the spread of further contamination, Integrated worked around the clock to remove the free liquid. Contaminated soils were accessed after impacted site structures were abated and demolished. In all, over 10,000 yards of PCB-contaminated soils were excavated, analyzed, loaded, hauled off-site, and disposed of during the course of this project. In addition, approximately 2,000 yards of material was abated (full enclosure and glove bag techniques) and disposed of as ACM waste in conjunction with the demolition of 12 site structures. The project was completed almost two months ahead of schedule to the complete satisfaction of both the Owner and the NYSDEC. Integrated Waste Special Services, Inc. _________ 5 fWSSSOQ RV1/1*M ASARCO ELP 0010406 PROJECT DESCRIPTION PROJECT: Durez Site Remediation, Sewer Cleaning North Tonawanda, New York CLIENT: Occidental Chemical Corporation CONTACT: Mary Backlas (716) 278-7616 VALUE: $1,700,000 DATES: August 1990 - May 1992 DESCRIPTION: Integrated cleaned over 48,000 feet of sanitary and storm sewer lines inside and outside of the Durez Chemical plant in North Tonawanda, New York. The lines, which ranged from two to 48 inches in diameter not only ran throughout the plant, but also through city streets to the Niagara River. This project was considered an uncontrolled hazardous waste site by the New York State Department of Environmental Conservation (NYSDEC) and required the development and implementation of site-specific Health and Safety, Work, Quality Control, and Contingency Plans. As prime contractor, Integrated was responsible for overseeing air monitoring, sediment removal and filter pressing operations, and disposal of resin which had accumulated in the over 48,000 feet of sewer lines remediated under the scope of this contract. A two mile long flume also was cleaned by Integrated as part of this contract. The materials which were removed from the sewer lines contained various hazardous chemicals which included benzenes, dichlorobenzene, trichlorbenzene, and many others which dictated that the job be performed under Level B (60% of the project) and Level C (40% of the project) PPE. All personnel assigned to the project were 40 hour trained and certified. Integrated was responsible for the documentab'on of waste streams as well as USEPA, NYSDEC, and Owner required logs and reports. Integrated worked efficiently with all various departments to ensure that all operations were conducted and documented in accordance with established regulations and the contract documents. The project was successfully completed on schedule with all sewer and storm lines cleaned and video-taped to the satisfaction of USEPA, NYSDEC, and Occidental Chemical Corporation. Integrated Waste Special Services, Iiic. ________ 4-6 iwsssooRvvi.es ASARCO ELP 00104.07 PROJECT DESCRIPTION PROJECT: Underground Storage Tank & PCB Transformer Removal Grand Island, New York CLIENT: U.S. Army Corps of Engineers Grand Island Nike Battery CONTACT- Joseph Salvatore (716) 297-0531 VALUE: $854,103 DATES: October 1992 - May 1993 DESCRIPTION: Integrated removed and disposed of six NIKE missile underground storage vaults, eight underground storage tanks, and three electrical transformers at the Nike Battery Site in Grand Island, New York. Each of the underground missile storage vaults was approximately 47,000 cubic feet in size. Demolition of the six missile vaults involved sampling the liquid in each vault; disposal of the liquid; puncturing the door of each vault to allow uninhibited groundwater flow; backfilling the vaults with an inert material; demolishing the surface structures associated with each vault to a depth of six feet below the surface; backfilling the excavations; covering the area with a four inch layer of topsoil; and revegetating affected areas with grass seed. Integrated also removed and disposed of heating fuel and gasoline product contained within the USTs prior to closure in accordance with applicable state and federal regulations. UST removal involved evacuation and cleaning of the UST interiors, disposal of the liquid contents, removal and disposal of the USTs, collecting and analyzing post-excavation soil samples, excavation and disposal of contaminated soil, and restoration of the former UST locations. The three transformers handled by Integrated in association with this contract were tested forPCBs, classified, and'disposed of in accordance with applicable regulations. The contract was completed on schedule and within budget to the complete satisfaction of the USACE's contracting officer. Integrated Waste Special Services, Inc. _________ 4-7 IWSSSOaRVVl-09 ASARCO ELP 00104-08 PROJECT DESCRIPTION PROJECT: Kenfield Apartments Lead & Asbestos Abatement Contracts (Phases I, II, and III) Buffalo, New York CLIENT: HUD CONTACT: William Quinn, Site Manager Buffalo Municipal Housing Authority (716) 855-6800 VALUE: Phase I -$1,696,869 Phase II -$2,389,210 Phase III-$3,419,079 DATES: Phase I - April - November 1993 Phase II -December 1993-June 1994 Phase III - June 1994 - May 1995 DESCRIPTION: Integrated was awarded three contracts to perform lead abatement, asbestos abatement, and interior renovation of 192 individual apartment units including connecting hallways and mechanical spaces Phases I and II involved the lead abatement of all surfaces including doors, walls, ceilings, stairs, and kitchen cabinets. Once abatement of all removable or strippable surfaces was complete, Integrated installed over 480,000 square feet of dry-wall which was designed as an encapsulation method to the remaining surfaces. Phase III involved the abatement of interior friable and non-friable asbestos con taining material (ACM). Glove bag and full enclosure/bulk removal abatement techniques were employed at the site to remove ACM floor tiles, ceiling tiles, and insulation. A specially-designed bead blaster with HEPA filtration system was utilized to remove the floor tiles. All abatement work (asbestos and lead) was done in full accordance with applicable state and federal regulations. The contracts were completed on schedule to the complete satisfaction of the client. CONTACT: Gil Tomei, Plant Manager (716) 286-0242 VALUE: $2,200,000 DATES: October - December 1992 DESCRIPTION: Integrated's Asbestos Abatement Division (Asbestos Removal Services, Inc.) was called upon to respond immediately to an in-plant fire, asbestos/ PCB cleanup project at Carbide Graphite's facility in Niagara Falls, New York. Once on-site, Integrated implemented immediate site controls and conducted a pre demolition survey of the affected structure. Due to the unusual circumstances surrounding this project, Integrated's project management team felt that a variance would be required to safely and efficiently perform the contracted scope of work. Therefore, a Design and Application Permit was prepared and submitted to the New York State Division of Labor - Engineering Services Unit After permit approval was received, Integrated began the containment and treatment of 40,000 square feet of burned-out roofing and siding materials coated with asbestos-containing fire proofing. Integrated also contained, treated, and cleaned PCB electrical switch rooms which were damaged during the fire. NOTE: Abatement operations involved removal of asbestos from exposed heights in excess of 30 feet. The area was successfully abated in compressed time frame which enabled the Owner to resume production in as short a time as possible. Integrated Waste Special Services, Inc. ________ 4-9 IWSSSOO.RV1/1-9S ASARCO ELP 0010410 PROJECT DESCRIPTION PROJECT: Asbestos Abatement & Demolition of Roswell Park Apartments Buffalo, New York CLIENT: Lehrer, McGovern, & Bovis for the Roswell Park Cancer Institute CONTACT: Joel Peacock, Construction Manager (716) 845-4352 VALUE: $1,282,803 DESCRIPTION: Integrated's Asbestos Abatement Division (Asbestos Removal Services, Inc.) conducted asbestos removal prior to demolishing a 14 story apartment complex within the campus of Roswell Park Cancer Institute. Because the 14 story, reinforced concrete structure was within close proximity to active cancer research and treatment facilities, dust suppression, vibration, and noise abatement procedures were of major concern to the client. Therefore, Integrated designed and implemented detailed Work, Safety, and Abatement Plans which governed the removal of approximately 27,000 cubic yards of asbestos-contaminated reinforced concrete from the site for disposal at a local landfill. Asbestos abatement involved the removal of all friable ACM prior to demolition. Friable ACM included, but were not limited to, thermal pipe and mechanical insulation, floor tile and mastic, and window caulk. Non-friable ACM also was present within the facility. All ACM removal was performed through a permit issued by the New York State Department of Labor under its Code Rule 56 which calls for extensive air sampling within and outside of the regulated work areas. This sampling was performed continuously by a third party project monitor throughout the course of the project All friable ACM was double bagged, labeled, sealed, and loaded into sealed, 100 cubic yard van trailers which were attached to the work area via a series of air locks kept continuously under a negative air filtration system. Non-friable ACM was handled with the demolition material in accordance with applicable regulations.. The project was completed on schedule within 30 days to the complete satisfaction of the client. Integrated Waste Special Services, Inc. 4-10 IWSSSOO RV1/1-CS ASARCO ELP 0010411 PROJECT DESCRIPTION PROJECT: Site Work Grand Island, New York CLIENT: Grand Island Central School District Sidway School VALUE: $400,000 DATES: July - September 1993 DESCRIPTION: Integrated was contracted by the Grand Island School District to perform site work at their Sidway School location on Grand Island. This project involved the removal and replacement of existing paving; abandonment of vaults; and removal of playground equipment. Also included were the design and installation of a storm sewer system, a handicap ramp with foundation, integral and stand-alone concrete curbs, sidewalks, and landscaping. During construction, Integrated observed that the majority of the parking lot needed to be stabilized in order to accommodate heavy bus traffic. Based on this observation, Integrated redesigned the parking lot accordingly and successfully completed the project to the client's complete satisfaction. Tiie contract involved the total demolition of a foundry with a gross floor space of over 1,000,000 square feet and over 75,000 tons of steel. The contract also required Integrated to conduct asbestos abatement and hazardous waste removal operations prior to demolition. Integrated also was responsible for the removal of a two mile concrete road and over two miles of railroad track which covered the two acre site. All operations associated with the successful completion of this contract were completed by the companies which make up Integrated Waste Special Services, Inc. The project was performed on schedule and within budget to the complete satisfaction of the Owner. Integrated Waste Special Services, Inc. 4-15 IWSSSOQRV1/1>96" ASARCO ELP 0010416 PROJECT DESCRIPTION PROJECT: GM Automotive Plant Demolition Lordstown, Ohio CLIENT: General Motors Corporation CONTACT: George Johnson (216) 824-6746 VALUE: 53,000,000 DATES: July 1994 DESCRIPTION: Integrated's Demolition Division, U.S. Dismantlement Corporation, completed a turn key project at GM's Lordstown, Ohio plant which involved utility disconnection and re routing; concrete patching and restoration; and demolition of equipment, utilities, process equipment, ovens and spray booths from the floor to the bottom cord of truss. By working around the clock with 150 men per shift and utilizing numerous pieces of specialty equipment, Integrated demolished and removed 6,000 tons of iron in 11 consecutive days maintaining an extremely aggressive schedule while achieving a high standard of safety. In addition to dismantlement, this contract also involved working with lead paint. Integrated identified problems areas within the plant and utilized appropriate PPE and engineering controls during removal. The project v/as completed on schedule and within budge to the complete satisfaction of the client. NOTE: U.S. Dismantlement Corporation's (Integrated's) crews were awarded a Certificate of Commendation by the National Safety Council for the safe manner in which they performed "very dangerous tasks at the General Motors Plant in Lordstown, Ohio". Integrated's site safety officer on this project also was awarded a Meritorious Service Award for his involvement with this project. The scope of work for this project ranged from simple architectural removal to complex structural demolition, including shoring. Integrated performed portions of this project while school was in session. This made scheduling and coordination with other subcontractors an essential part of the contract. Integrated completed its scope of work within the client's scheduled time frame which allowed the remainder of the project to remain on schedule.
