Notification, notice is hereby given that your exposure of Asbestos fibers was in excess of the permissible limits on September 6, 1977. Eight (8) each - thirty (30) minute personal air samp les were taken. The average asbestos fiber count for the eight (8) samples was 2.36 fibers longer than 5 micrometers, per cubic centimeter of air. During the time of exposure you were using an approved respirator.
