The original Physical Examination Form shall be sent to the Houston Safety Office. A copy shall be retained at the job site. 2. All records for monitoring of dust samples and the laboratory report shall be sent to the Houston Safety Offiee. A copy of the reports shall be retained at the job site. 3. Notification in writing of over-exposed employees to asbestos shall be made immediately to the employee.
