Vinyl Chloride Panel Chemical Manufacturers Association 1300 Wilson Boulevard Arlington, VA 22209 Dear Dr. Shah: Enclosed is a final inventory of 24 boxes of study materials received fromNSR on May 11, 1995 and July 13, 1995 which appear to be unrelated to our VCM/PVC mortality study. Because most, if not all, of the boxes contain personal identifiers, and at least one contains medical information, we have maintained them in our locked storage facility. However, now that we have ascertained that the materials are unrelated to our study, we request your advise on how we should dispose of them. Two obvious options include shipping the materials back to ENSR via United Parcel Service (estimated cost, based on average weight of 30 pounds per box, is $20.84 per box plus a $5.00 pick up charge, plus our charge of $200 for packing and labeling); or sending the boxes to a records destruction facility (estimated total cost $300). If ENSR should choose the latter option, however, 1 would require a letter from an official of the company authorizing us to destroy these records and assuming full responsibility for their destruction. Please let me know at your earliest convenience whether ENSR selects either of these options, or proposes some other solution. Although there is no urgency, I would like to free the storage space before the end of the summer, if possible. Please feel free to call if you have any questions or require additional information.
