sm SmartTrunk CONTRACT FOR SERVICE
THIS CONTRACT is effective this 6th day of November, 1996, between Southwestern Bell Telephone Company (SWBT) and City of San Antonio (Customer) as follows:
1.Southwestern Bell Telephone Company agrees to install telephone service consisting of SmartTrunk service at the following location 506 Dolorosa, San Antonio, Texas
2. Customer agrees to accept and pay for such service and for any additional service or modifications thereof as may later be agreed upon to be installed, at the rates and terms established in the SWBT's tariffs, and further agrees to the rules and regulations set forth in said tariffs. If a conflict between this contract for service and the tariff occurs, the tariff will govern in all instances. A copy of the tariff is available upon request.
3. The service term for the SmartTrunk Interface will commence on the date service is installed and shall remain in effect for a minimum term of 60 continuous months and thereafter until terminated.
4. The Customer elects/does not elect to defer installation and nonrecurring charges over a period of _months, and if deferred agrees to pay the applicable deferred payment charge at the tariff rated.
5. The terms and conditions of this contract are equally binding upon the parties named herein, their heirs, assigns and successors in interest.
