The Amendment authorizes costs for these additional services on a time and material basis, including reasonable and necessary travel expenses, not to exceed $12,500.00. This Amendment brings the total costs of this agreement to an amount not to exceed $449,895.00. Payments for services rendered under this Amendment will be made following receipt by CMA of itemized invoices that describe services rendered, charges for such services, travel and other direct expenses. By this Amendment, CMA replaces Wendy K. Sherman for Hasmukh C. Shah as the Vinyl Chloride Panel Manager. All future correspondence under this Agreement should be directed to Ms. Sherman. By this Amendment, the above -referenced Agreement is modified to be subject, and to be construed under, the laws of the Common Wealth of Virginia, United States of America. Actions under this Agreement shall be brought in any court of competent jurisdiction in the Commonwealth of Virginia. If the terms of this Amendment meet your approval, please sign two copies of the Amendment, retain one for your files and return the other to Ms. Sherman. Juiy 3, 1997 Dr. Hasmukh Shah Manager, Vinyl Chloride Panel Chemical Manufacturers Association 1300 Wilson Boulevard Arlington, VA 22209 Dear Dr. Shah: With this letter I wish to propose that Applied Epidemiology, Inc. serve as consultant to CMA's VCM Panel, to oversee the development and conduct of Dr. Carlo Tamburro's proposed study, "Brain Cancer Occurrence in Vinyl Chloride Exposed Chemical Workers." Specifically, we would provide the following services: Critically review and comment on the study proposal and study protocol; Participate in meetings with Dr. Tamburro's staff and CMA representatives; Participate in telephone and conference calls with the VCM Panel and Dr. Shah and bis staff as needed; Compile documentation of the oversight process for CMA's records; Assist Dr. Tamburro's staff with technical support and guidance; To the extent possible (under confidentiality agreements) provide to Dr. Tamburro's staff data from the VCM mortality study; Critically review any progress reports, draft final reports-andrmanu^ripts generated by the study; Provide CMA with ideas and opinions on overall study quality and conduct. For these services, I estimate that total costs for professional and staff fees and expenses will be $12,500. This estimate includes travel expenses for two trips to Louisville to meet with Dr. Tamburro and his staff. Please do not hesitate to call ifyou have any questions or require additional information.
