ILLINOIS 60604 ^TIO* v A% Enclosed is a supply of National Pollutant Discharge Elimination System (NPDES) Discharge Monitoring Report Form T-4Q to be used in reporting to the U.S. Environmental Protection Agency during the life of your permit. Discharge Monitoring Report T-40 is to be completed in the following manner: 1. Use one Form per month per point of discharge (or per group of discharges limited as a unit). 2. Enter "Monthly averages"; also show Maximum and Minimum values which occurred during the month. (See page 3, item No. 7) 3. Enter in uniform order (as conditioned in your Permit) from month to month each parameter specified in your permit to be monitored that are not already preprinted on th- Form. 4. Retain the completed copies of monthly Forms until the close of the quarterly report period, then send three completed monthly Forms (for each discharge or group of discharges) to the U.S. EPA. Copies to the State must follow State reporting requirements as specified in the permit. Be sure to maintain a copy of completed Forms for your records. Example 1 Permit with three points of discharge, separately limited: Complete and hold a Form for each of the three discharges as follows: January 3 Forms, completed (hold) February 3 Forms, completed (hold) March 3_ Forms, completed By April 28: 9 Forms submitted Etc. GENC 004411 Example 2 Permit with three points of discharge, limited as follows: 001; 002 and 003 in combination. Two (2) forms are then required to be completed each month, as follows: January - 2 forms completed (hold) February - 2 forms competed (hold) March By April 28: - _2 forms completed 6 completed forms submitted. Etc. Example 3 Permit with three points of discharge, limited as a group. Complete and hold one form for the group as follows: January - 1 form completed (hold) February - 1 form completed fhold) March - 1_ form completed By April 28: 3 forms completed and submitted. Etc. Items 7-12 apply to all r>e~r,i -1 -r- . 1. In the box marked "ST" enter two letters to re^reseat the "c.-iC' i, which the discharge is located. These letters arn rhe =s fu' first two letters of your permit number. 2. In the box labeled "PERMIT MUMBER" enter tha "-digit permit -nr--" It is not necessary to repeat the 2-letter State cede. GENC 004412 3. In the box marked "DIS" enter the serial number of the point of discharge for which the Form is being completed. Serial numbers are 3-digit numbers of the form 001, 002, etc.. Please enter leading zeroes. If the Form represents a group of discharge points, enter the lowest serial number in the group. (e.g. If the Form represents the total discharge from points "002", "003", and "004", enter "002".) If your facility has only one discharge point, enter "001". 4. The boxes marked "SIC" and "LATITUDE-LONGITUDE" require no entries. 5. "REPORTING PERIOD." The dates of the first and last days represented by each form must be entered numerically. If the month or day is lower than 10, it must be entered as 01, 02, 03, etc. Leading zeroes must be filled in. Example: A Form representing the month of January 1975 would read, "750101 to 750131". 6. The column marked "PARAMETERS" is being used to identify the names of chemical or physical properties being measured. The computer recognizes these names as 5-digit numbers, known as parameter codes. These parameter codes must be entered on the form before the data can be processed. Following is a list of the most frequently used parameter' codes. If you are monitoring parameters not included on this list, merely write in the name of the parameter, ignoring the code. You may, for your own use, write in the names along with the numbers of the parameters which do appear on the list but vM-' n/o not been preprinted on the form. 7. To determine the arithmetic mean of loading, the daily f7cv, . i i^llion gallons per day, mu~t he multiplied by the da^lv 'once u t r -11 >, Li ng/L, multiplied by 3.79 to yield the daily load in kilo trims. BOPs, Suspended Solids, and Fecal Coliform: Under "Quantity" enter the arithmetic mean* of the loading determined from the samples collected during the calendar month in the "average" column; in the "maximum" column enter the highest arithmetic mean of GENC 004413 4 the loading determined from the samples collected for any calendar week during the calendar month. If a week ends in a new month, that week's data is to be reported in the new month. All data under columns headed "Quantity" must be entered using the metric system. Flow is the only exception and is to be entered in million gallons per day. Your permit shows loading limitations in both English and Metric systems. Multiply pounds by 0.453 in order to obtain kilograms. Under "Concentration" enter the arithmetic mean of the concentrations of the samples collected during the calendar month in the "average" column; in the "maximum" column enter the highest arithmetic mean of the concentrations of the samples collected for any calendar week during the calendar month. If a week ends in a new month, that week's data is to be reported in the new month. For fecal coliform bacteria the geometric mean* must be used for reporting both monthly and weekly averages. pH and Residual Chlorine: For pH and Residual Chlorine enter under the "maximum" and "minimum" columns the extreme values observed during the calendar month. Other Parameters: For other parameters enter under the "maximum" column the extreme value observed during the calendar month. In general, loadings are reported under "Quantity"; concentrations are reported under "Concentration". For pH, report under "Quantity". IMPORTANT NOTE: Although the monitoring is to be reported on a calendar month and calendar week basis, the permittee must recognize that permit conditions require compliance during any 30 consecutive day or any 7 consecutive day period. Any violation occurring between any two consecutive months or two consecu tive weeks must receive immediate attention from the permittee and may be cause for enforcement action. GENC 004414 -5- *Example: seven samples collected during a period of seven days are as follows: 30, 43, 25, 20, 27, 28, 22. The arithmetic mean (AM) of the above samples is given by: AM = 30 + 43 + 25 + 20 + 27 + 28 + 22 7 = 28 The geometric mean (GM) of the above samples is given by: GM a/'J/'30 x 43 x 25 x 20 x 27 x 28 x 22 Take logarithm of both sides of the equation. Log GM * 1/7 (log 30 + log 43 + log 25 + log 20 + log 27 + log 28 + log 22) - 1/7 (1.478 + 1.632 + 1.398 + 1.301 + 1.432 + 1.448 + 1.342) - 1.433 Find the geometric mean from the logarithm table, anitlog 1.433 27, therefore GM - 27 8. When entering data, be careful to show decimal points clearly and DO NOT USE COMMAS on large numbers. 9. In the column marked "NO. EX" enter the number of times the maximum and/or minimum was exceeded. For pH enter the number of times the allowable minimum or maximum limits were violated. 10. The column marked "FREQUENCY OF ANALYSIS" is designed to record how many times each parameter was measured. It appears logical to enter "weekly" or "monthly", but the computer requires that the data be entered In the form of number of samples per number of days. Examples: once per week would be entered as "1/7;" monthly, as "1/30;" daily as "1/1;" twice daily, as "2/1;" three times per week "3/7;" etc. If the measurement is continuous, enter "CONT.". GENC 004415 6- - 11. The column marked "SAMPLE TYPE" is designed to record how the samples were collected. If they were grab samples enter "GR;" if they were composite samples, enter the number of hours composited. (A 24-hour composite would appear as "24", and an 8-hour composite as "08", etc.). If the parameter is measured continuously, enter "NA" for not applicable. 12. In the upper left hand corner of the T-40 Form, please enter the name, address, and telephone number of the treatment facility. Inasmuch as our initial supplies of the Reporting Form have been extremely limited, we may not have been able to include the total number which you will require during the life of the permit. As your enclosed supply dwindles, please either: 1. Write or call this office for an additional supply. We will furnish forms as soon as possible; otherwise, 2. Photocopy one of your blank forms in numbers sufficient for your reporting needs. ?
