Section I - INTRODUCTION
PURPOSE OF THE INSPECTION
EPA Region 6 inspectors Uma Lad and Kerri McCabe arrived at the Cypress Operating, Inc. / Haydon #1-A SWD System, located off County Road 415 at 33.254492, -93.953885, at 1:28 PM on 01/12/2026 for an unannounced inspection. No facility representatives were present at the time of the inspection. The inspection was conducted by the authority of the Clean Water Act (CWA) to determine compliance with Environmental Protection Agency (EPA) regulations. There were no other federal, state, or local agency representatives present at the time of the inspection. FACILITY DESCRIPTION
EPA conducted an inspection at Cypress Operating, Inc. / Haydon #1-A SWD System, which consists of four (4) storage tanks, one (1) separator, one (1) production well, and one (1) saltwater disposal (SWD) injection well. Three (3) of the four (4) storage tanks appeared to be out of service or not in use.
Section II - OBSERVATIONS
I observed the facility, associated signage, production well, SWD injection well, storage tanks and secondary containment; see Appendix 1 (Photograph Log), Photo No. 1 through No. 4. For additional GPS location and photo metadata, see Appendix 1. I observed loose oil around the injection pump, see Photo No. 5. I observed three (3) storage tanks outside secondary containment that appeared to be "out of service" or "not in use" and did not have piping, see Photo No. 6. I observed indicators of past or historic saltwater discharge based on a flow path from the facility, presence of salt crystals, and dead vegetation, see Photo No. 7 through No. 9. Section III - AREAS OF CONCERN
No closing conference was held due to no Cypress Operating, Inc. staff present during the inspection. Areas of concern at the time of the inspection are: Loose oil around the injection pump; Indicators of past saltwater discharge based on visible flow path from the facility.
Section IV - FOLLOW UP
Inspector will conduct follow-up phone calls with the company representatives over the coming weeks and months to ensure areas of concern are properly addressed.
