Mr. Spence's employment record including job titles and dates of transfer. 2. Written job descriptions of each job done. (It would be preferable if the job descriptions were pertinent to the time of employment of Mr. Spence.) 3. Dates of physical examinations. 4. Sampling data by OSHA and/or company hygienists for asbestos. Also any other sampling data (for other occupational exposure such as silica or oil mist exposures) done in areas in which Mr. Spence had worked (excluding noise surveys). 5. A Material safety data sheet (MSDS) for Renite and Kevlar. 6. The name and title of a foreman or the nurse that may have known and worked with Mr. Spence. Perhaps they could say whether the claimant was a smoker and whether or not he had any hobbies that would contribute to his dust or fume exposure. 7. Confirmation of whether or not monochlorobenzene was used in your quality control department and whether 25% ammonium formate was used to spray hot molds in the 1960's, (per Division of Safety and Hygiene survey reports.) Owens-Illinois Company Page 2 Ns.
