Spoke to Ms. Dee Netter, Environmental Section, who referred me to Mr. Norman L. Rearick, Chemist.
Mr. Rearick returned my call. He hot the report and has tried to substantiate the reports, especially with the electric companies. He has no record of a spill on 10/6/82 on Sullivant Avenue, nor does he have ony reporl for 10/10/82. The only record of a spill he has is on 9/10/83 at 2879 Sullivant Avenue. The capacitor exploded and dumped S.l gallons of PCBfluld. There were no Injuries, there was no one around at the time. Mr. Rearick was loter sent to the site, since EPA, Ohio, hod objected to the original cleanup of the tiding on the house next door to the spill. (The spill was in an alleyway, so the fluid hod spilled an vegetation, earth, and the side of o house. EPA, Ohio, hod approved of their cleanup of the vegetation and earth. They removed approximately 5 barrels of bushes and earth and replaced same. The house siding was cleaned with solvent/detergent.
Mr. Rearick went to the site after EPA, Ohio, disapproved of the house siding cleanup. They cleaned it again and received EPA approval.
According to Mr. Rearick, there were no injuries to anyone.

Corporation, however, American Electric continued to store PCBs In the main storage area outside the building. American Electric Company did not report the sale to EPA. Initially, arson was suspected, however, this was never proven.
One or two 3,000-oallon tanks containing PCBs were located inside the building. These tanks did burn. The combustion was supported mainly by petroleum. The main storoge area was outside the building and contained a 50-60,000 gallon tank. This was kept wet by firemen and did not burn or leak. The clean-up of the site was supervised by EPA. The contamination assessment of the neighborhood found light traces of PCBs but below Federal limits. Two men were burned in the explosion, one seriously. There have
never been any complaints of PCB-related injuries, even by the two men who were burned.

Spoke to Jo Brooks on 2/26/86. She explained that a truck (poorly loaded) was
corryina transformers. Some of these transformers fell off the truck, ruptured, and spilled (6-10 gals, of fluid). She sold thot she hod been on the job only obout a week and remembered the Incident well. It did make the papers and coused quite a stir. She hod a picture showing the truck driver (who hod no shirt on) having his forearms washed down
by two men in decontamination suits. She did state that there were absolutely no injuries and that there were no reported burns or other adverse health effects by the truck driver.
AoogQgQfltom a transformer bounced off a truck onto the freeway, spilling approximately 13 gallons PCB-laden oil on freeway. Holf of the remaining transformers on the truck leaked an undetermined amount of PCB. The eastbound lanes of the
freeway were sealed for a five-mile stretch between the Interstate 205 interchange ond Foirview for about 24 hours. About a quarter-mile stretch of the roadway was resurfaced. (Officials reported the concentration of PCBs in the oil was 300 ppm.) It
was possible that several hundred motorists drove over the oil material. The spill covered an area about 250 feet long, but passing cars probably spread the PCBs more than o mile down the freeway. Oregon Highway Division crews spread 25 50-lb. bags of absorbent moterial over the spill to remove as much as possible from the roadway. Authorities said the leaking transformers would be loaded in steel containers and that the

truck driver, who was cited for carrying an insecure lood, would be ollowed to drive them to an authorized disposal site in Nevada. By Saturday evening, three people who sold they might have been exposed to the oil had contacted the Multnomah County Sheriff's Dept., one of whom complained of a headache and sore throat. Motorists who were on the roadway in the area of the spill were urged to run their vehicles through a car wash, avoid any direct contoct with the oil, and have their vehicles tested by EPA officials after they had been cleaned.

ot 2700 N. 71st St., Milwaukee, Wl. A tree branch fell on a bank of capacitors and some were cracked and discharged liquid. Wisconsin Electric (WE) hod the area completely clean by 6)00 aun. the next morning. DNR was notified, WE took it upon themselves to talk to the area property owners and notify them of this Incident. One lady, who lived at 276S N. 70th St., said thot she was standing on her porch at the time of the incident. She was upset over the fact that she may have been exposed to PCBs. (The lady was a nurse.)
According to Dr. Lingle, there were no Injuries. The NRC report listed one injury and the only thing Dr. Lingle can think of is that they are counting this lody's report as being injured/exposed.

Spoke to Mr. Walker on 2/25/06. Mr. Walker stated thot outside contractors (steelworkers) were working on the floor level. They were making renovations, taking out old machinery, overhead electrical, etc., when a roof leak developed. This leak
dripped on six (6) men. Upon checking the roof, the transformer located on the roof was noted to be leaking a fluid, hod mixed with some water on the roof, and had dripped down on the floor area on these six men. Ford Motor Company examined these men in their medical facilities, had the men dispose of their clothing and shower. Upon examination, there were no burns or octual injuries. The men were reimbursed for their clothing and told of their exposure, as was the company they worked for.
A letter was received by Ford Motor Company from the Michigan Dept, of Public
Health telling them that they (Ford Motor Co.) hod done everything according to the rules (regarding the cleanup). As far as Mr. Walker knows, there have been no health problems with any of the six men exposed.

Record of Conversation with Referent(s): Spoke to Mr. Peters an 2/25/86. He stated that this incident was the only PCB-
related incident in the 4-5 year history of the plant. (Ceno's Is a food processing plant.) He stated that this incident was quite minor) that a transformer did blow up, and one person was sprayed. That person was sent to the doctor as a precaution, however, he suffered no bums or other ill effects. No food wos contaminated. Mr. Peters checked his OSHA log for this Information and soid that there was only one person Involved, not two as reported In the NRC and the EPA reports. He also stated that they did remove some earth, but that there was no soturotion. They refilled the area with clean dirt/earth. The 16.5 ppm reading was possibly taken by the State of Ohio EPA) Genos hod no information on the reading. As of this date, no other octlon has been token on
this cate. There are no on-going problems.

written Inquiry stating the reasons for the request, and what use/purpose would be made of the Information. He stated that he could not see the reasons for such Investlgatlon/tnqulry ond that he had read that this was "just the tip of the iceberg," that there were actually many more incidents -- that all the states had not been reported.
On 7 March, Ms. Nancy Tidona, External Affairs Dept, of Dupont called in response to Ms. Rlgelon's letter to Mr. Stowell of 28 February. She stated that she hod checked
the file for this incident. The report reod that there was a trial process run and that the olr was tested ot I/S of I part per billion PCB. There were no injuries. As far as Ms.
Tidona knows, they have hod no PCB accidents. (This incident was so small that it was not reported to the NRC until several months after it had occurred when it was recognized that an accident could have happened.)

Ms. Rlgelon, Everest Consulting Associates, spoke to Ms. Lump who advised me to write for the accident report. She stated that there was no record of this in her office at this time, however, due to the fact that the incident took place a few years ago, I could probably get a copy of an Occident reporl involving PCB on that dote -- 5/1/83.
Ms. Rlgelon has written for a copy of this report from the Dept, of Public Safety, Accident Records, P.O. Box 6638, Phoenix, Arizona 85005, enclosing $10.00 for the
written report. Phonet (602) 262-8011.
On 7 March, Mr. Ed Roy, Supervisor, Arizono Highway Patrol, called to "try to shed more light on the subject," however, he said he would need the name of the "carrier" and/or the name of the truck driver in order to locate any information. This wos not provided on the NRC report. He then referred me to Mr. Rick Saunders, Esq., Arizona Dept, of Public Safety, Legal Office, POB 6638, Phoenix, Arizona 8500S (602) 262 8250. Ms. Rlgelon called Mr. Saunders, however, he was not available. Ms. Rlgelon sent a certified letter today requesting this information. As of 26 April, there has been no response from Mr. Saunders.
On 23 April, Ms. Rlgelon called Mr. Saunders' office again and wos referred to Oon Harvey (615) 976-8386. He wos not in the office but would return the coll. On 6/28 Ms. Rlgelon called Mr. Harvey again and was told he wos not In the office but would return the call. No response os of 6/29/86.

Spoke to Carol Russell who remembers the accident. (It happened in front of their previous locotion.) She stated that her supervisor actually called the authorities since he knew that the transformer ruptured. Ms. Russell said that a car (traveling at about 80 mph in a 3S mph zone) struck a light pole, knocking down a transformer and rupturing it. (The pole octuolty fell and electric wires were on top of the auto. The "jaws of life" were used to remove the man from the auto.) As Ms. Russell recalled, the man had only
a minor cut on his hand and was walking when he got out of the auto. She said that there was a small amount of substance, identified as a PCB-containing liquid, draining from the transformer. Authorities covered the road with sand, but never (as of June 1984) did any
digging or earth removal. They said that the sand would take care of the problem.
The above was the only possible injury involving this incident.

Spoke to Mr. Onishi on 2/25/85. He has a copy of the exhibit, "Chemical Releases in Which Injuries Occurred," ond has checked with the electric company involved and can find no case where they have any record of a PCB-related injury on the dates given in the EPA report.
Mr. Onishi has spoken to Bryce MacDonald and also to Dan Maxim.
Mr. Onishi sent a copy of a letter he wrote to their attorney regarding this report and his findings. He did stote that he doubted the validity of the report done far EPA.

Ms. Rlgelon spoke to referent on 4/26/86. Referent stoted that he wos driving a tractor troiler with a dump bed on Route 17 when he rounded a horseshoe curve and
swerved off of the pavement to ovoid hitting an oncoming automobile that was traveling toward him In his lane. He missed the outomoblle, however, he was unable to get his rig back on the pavement -- his rig turned over, he was thrown through the windshield, and the load spilled onto the roodway. He was hurt in the accident, however, he remained at the scene for 4-5 hours until his company (Chemicol Wastes Management, Inc.) arrived
and had the cleanup underway. He was then transported to the hospital, received treatment to cuts and bruises, and wos discharged. According to Mr. Robson, his injuries
were due to being thrown through the windshield. Mr. Robson stated that personnel from the local Civil Defense were among those on the scene doing the cleanup. He stoted that
soli was dug up and hauled away. He described the cleanup as "very efficient."
Mr. Robson Is no longer working for Chemical Wastes Management, however, he is still hauling hazardous chemicals. (He did not offer the nome of his present employer.)
He was very concerned about being incorrectly quoted and I assured him that thot would not happen.
