A pipefitter working at South Charleston suffered a back sprain when he bent over to pick up a 4' piece of 1 1/2" pipe. The employee experienced back pain as he bent over to pick up the pipe. There was no accident or event such as a slip, trip, fall or overexertion. The employee was absent from work for three (3) days. This Is the first LWC for EHTS In 1987. Recordable Injuries - Construction had four (4) Recordable Injuries in March. All four were back Injuries. There was one (1) Recordable Injury at the Technical Center In March. A clerk in the Polyolefins Engineering Department lacerated her left hand with an x-acto knife and required five (5) sutures. Detailed descriptions of these five (5) Recordables are provided In the attached BLS/OSHA Injury and Illness Sumnary. Recordable Illnesses - One (11 Recordable Illness was reported in March. An EKTS Engineering employee was diagnosed as having asbestos related lung disease. A second Recordable Illness that involved exposure to an Insecticide in Building 3001 In February was reported late and is reflected In the March monthly totals. The Technical Center now has a total of ffve (5) Recordable Injuries and seven (7) Recordable Illnesses and Construction has eleven (11) Recordable Injuries and tero (0) Recordable Illnesses. OFF-THE-JOB SAFETY SUMMARY Four (4) OTJ Disabling Injuries were reported in March. Construction had three (3) and Polyolefins R/D had one (1). The attached Off-the-Job Sumnary includes a description of each injury. The Technical Center now has a total of twelve (12) OTJ Injuries, seven (7) involving Tenant Division employees and five (5) to EMTS employees. The Construction Department now has a total of three (3) OTJ Injuries. BACK INJURIES Eight (8) or 73% of Construction's eleven (11) Recordable Injuries YTD are back injuries. Construction Management has alrea<(y implemented a back Injury prevention program that includes formal training in proper lifting techniques for all Construction employees. There have been no recordable back Injuries at the Technical Center In 1987. UCC 022385 UC-4064 ** 2- - accipental property LOSS OVER $5,000 On February 14th, the Construction Department installed two 440V-120V/208V TR8 transformers in the basement of Building 2000. These new transformers replaced equipment that was installed In 1982. Wires on the old transformers had been labeled incorrectly; consequently, the new transformers were wired Incorrectly. When the transformers were energized, several pieces of electrical equipment in Building 2000 were damaged. Total estimated damage came to $9,934.71. Construction and Services conducted a formal Investigation and have developed and Implemented a corrective action plan to ensure a similar accident can be avoided. PROACTIVITY FACTOR This month we have Included in our report a proactivity factor for the four major Technical Center groups. This factor is based on the formula listed below and can be used to monitor how proactive reporting in the form of unsafe condition reports, safety suggestions and near-miss incident reports Influence safety performance. Negative or reactive performance is reflected In terms of medical reports and accident reports for accidents with property damage and/or material loss. The theory is, "as the proactivity factor goes up - the injuries go down." This code comes from the CAP Safety Analysis Report (copy attached) that is completed and sent to the CAP - HSAEA Group by each division each month. All Lost Workday Cases and Recordable Injuries charged to the Technical Center and Construction year-to-date have been classified as "Minor." TECHNICAL CENTER CORPORATE AUDIT The Technical Center has been scheduled for a Safety and Health Compliance Audit by the Corporate Community and Employee Health, Safety and Environmental Protection Group. Probably due to exposure to asbestos dust. 12. INJ 03/09 POLY-Engineering 2000 Clerk 0036R Minor RI-F (Other) Shield holder slipped as the employee was replacing a protectlve shield on a x-acto knife, lacerating the left palm.
