Sharp - PER The 2nd Quarter Safety and Housekeeping Inspection for Departments 44, 45, 46 was conducted by the inspection team of Jim McPhail, Mike Burkhalter, Bill Recknor and B. J. Sanders on April 16. The department was given an "outstanding" rating. The following items were noted: Safety 1. AVIP is up-to-date 100%. G 2. Interlock bypass procedure is documented and followed. HHMA and interlock bypass procedure was reviewed at a recent safety meeting. 3. 600 lb. steam lines to CTW pump turbines need back welding. TFE ball valves in live steam service needs replacing with gate valves. 4. Coupling guards on 44P102-1&2 need repair or replacing. 5. No grating over ditch on SW corner of reformer. 6. Bottle rack or adjacent reformer roof drain on SW corner of reformer needs relocating. 7. Safety shower head piping on north side of CT-3 needs replacing. Housekeeping 1. Nitrogen sign at 44P104-2 is missing. 2. Fire extinguisher No. 44-5 needs cleaning. 3. "Temporary scaffold on 2nd level of reformer on SW corner needs removing. 4. Spare screens for CT-3 need a more permanent storage method. c 5. General clean-up needed on north side of CT-3 near carpenter shack and old smoke shack. SC 000314 LAM001511 6.
