Per OSHA PSM standard 1910.119, closure requires documented response to the recommended actions. All of the recommendations, descriptions of intended and actual actions taken, actual and/or expected completion dates for those actions, and the person responsible are attached. Chlor-Alkali II Of the 198 recommendations made, 197 have been completed and closed. One action item remains. This action item involves developing loop test procedures for critical instruments. The division instrument department is in the process of developing such procedures for critical instrumentation in the division. Due to the large amount of work involved, the expected completion date has been set in 1997. This recommendation will carry over into the 1997 audit recommendations. Caustic Of the 68 recommendations generated, 2 remain open. Both of these items are associated with the Electrical Reliability Audit. These two recommendations are both costly and difficult to implement. The department's decision has been to extend these two until the upcoming reliability audits take place.
