Including the totals for the Technical Center site and Construction at the top of the page, are based on the new OSHA/BLS Recordkeeping Guidelines for Occupational Injuries and Illnesses. We used these new guidelines In July, but the reclassification of injuries and Illnesses from January 1 through June 30 was not completed until this month. After the reclassification of all medical cases, the Technical Center and Construction ended up with 39 Recordable Injuries and 13 Recordable Illnesses. ' o Impact of Reclassification - Technical Center and Construction recordable injuries for the period January through June 30 doubled as a result of the OSHA/BLS reclassification. For the entire Technical Center, Including Construction and tenants, we had a total of 9 Recordable Injuries under the old recording criteria, after reclassification we had 20. Prior to reclassification EMTS functions had a total of 9 Recordables and following reclassification we had 18, a 100* Increase. A summary of this reclassification activity Is attached. o Negative Trend - July and August Performance - the high number of Recordable Injuries in July and August were not totally the result of the new OSHA/BLS classification criteria. In fact, since July 1, the Technical Center and Construction have experienced 9 serious Injuries that would have been recordable even under our old criteria, 5 of these occurred at the Technical Center and 4 in Construction, o OSHA/BLS Recordables YTD - To bring all functions up-to-date with their OSHA/BLS Recordable Injury experience year-to-date, we prepared a sumnary report that Includes a brief description of all Recordable Injuries for all functions through August, 1986. o August Injury Performance - In August we had a total of five Recordable Injuries, each of these Injuries Is described In the attached Recordable Injury and Illness summary. Our August Recordable Injury experience Is: Services -3 R/D (EMTS) -1 Construction -1 o Year-to-Date Recordable Illness Summary - Our Recordable Illnesses for the period January 1 June 30 doubled as a result of reclassification. All functions had a total of 5 Recordable Illnesses prior to reclassification and we ended up with 10. For EMTS we had 5 prior to and 8 after reclassification. Trn. Pipefitter Oper. Tech DESCRIPTION OF ACCIDENT AND INJURY Fracture of right ankle. The employee got out of a pick-up truck and was walking toward the back of the truck when he stepped on a slightly uneven surface. He twisted his ankle causing the fracture. He did not fall to the ground. Medical treatment and restricted work were required. 1 1/2" laceration of right thumb. The employee and a co-worker were changing the fiber chopping blades on a compound resin mold machine. His hand inadvertently struck against one of the fiber chopping blades. The employee required sutures {medical treatment). Foreign bo<ty In the eye. The employee suffered a wind-blown FB in the eye that required medical treatment. Laceration of left forearm. The employee was attempting to help a co-worker who was carrying a heavy metal box. The box slid down his arm and resulted in the laceration. Sutures {medical treatment) were required. Irritation of both eyes. Employee was disconnecting a hose on the discharge side of a pump. The line had pressure on It and acetonitrile sprayed in his eyes and face. The employee aggravated an existing back condition from lifting boxes of magnetic tapes that weighed 35#. She required medical treatment and restricted work. Allergic reaction to unknown substance. The employee suffered respiratory and eye Irritation from airborne material in his office In Building 2000. The employee required medical treatment and restricted work. 2 burns of the left foot. The employee had spilled chemicals on his boots (probably acrylates) several weeks earlier. Instead of disposing of or decontaminating the boots, he placed them In his locker. He did not wear the boots again for five weeks. When he put the boots back on and wore them for a while he noticed pain In his left foot. He required medical treatment and restricted work. Bldg. 2000 on 8/1]1 1276A DESCRIPTION OF ACCIDENT AND INJURY Small pleural plague due to asbestos exposure. Red rash on both arms from his elbows to his hands. Possibly due to fiberglass that fell in a vessel In which he was working. 1 burns and Irritation of both hands, arms and abdominal area. He was exposed to acrylate vapors while charging a reactor. He was pumping HEA from 55 gallon drums and was wearing all required safety equipment. Medical treatment and permanent restricted work required. Low back pain. Employee moved furniture all day and aggravated a low back problem he has had for 15 years. Medical treatment and restricted work were required. Allergic reaction to unknown substance. The employee suffered respiratory irritation from exposure to unknown material in Bldg. 2000. -3- UCC 022393 NO. He was using a metal plate clamp to move steel plate. The clamp came loose and struck him in the mouth. The clamp was improperly hooked. Required medical treatment. Embedded foreign body (stainless steel sliver) at the base of the right thumb. The employee was handling a piece of 16 gauge stainless steel pipe covering when the sliver of metal went In her hand. Required sutures, medical treatment and restricted work. The employee was wearing rubber gloves. Foreign body In right eye. After going home after work on June 30th, the employee noticed a foreign body in his right eye, it Is not known If the FB got in his eye at work. Cutting 16 gauge stainless steel pipe covering. Attempted to turn a large piece over and lacerated his hand. He was not wearing gloves. Sutures (medical treatment) required. 2" burn of surface of right hand. Mixing insulation cement In a 10 quart bucket. Using hot water from a line that was steam traced to mix cement. Hot water splashed on his hand resulting In burn. Required medical treatment. Injury to right Index finger. Using a 4 1/2" electric grinder to grind on a 3" piece of stainless steel pipe. The grinding wheel struck against his finger and resulted In a laceration. Sutures (medical treatment) was required. Fracture of left thumb. The employee was unloading a desk from the bed of a truck and the desk dropped on his finger.
